摘要
目的观察麻杏石甘汤加减治疗小儿病毒性肺炎风热郁肺证对症状积分及肺功能的影响,为临床诊治工作提供参考依据。方法将医院2018年10月—2019年9月治疗的小儿病毒性肺炎风热郁肺证100例按照随机数字表法均分为两组,对照组50例采用常规西药治疗,观察组50例在对照组基础上采用麻杏石甘汤加减治疗。比较两组患者的治疗总有效率,临床症状积分,临床症状消失时间,肺功能及不良反应发生情况。结果观察组总有效率显著高于对照组(P<0.05)。治疗前两组患儿的发热、咳嗽、喘息及肺部罗音积分比较差异均无统计学意义(P>0.05),治疗后两组以上各症状积分均显著降低,并且观察组显著低于对照组(P<0.05)。观察组发热、咳嗽、喘息消失时间及住院时间均显著短于对照组(P<0.05),而肺部罗音略短于对照组,差异无统计学意义(P>0.05)。治疗前两组患儿的用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、最大呼气流量(PEF)比较差异均无统计学意义(P>0.05),治疗后两组以上各项均显著改善,并且观察组改善情况优于对照组(P<0.05)。两组均无严重不良反应发生。结论麻杏石甘汤加减治疗小儿病毒性肺炎风热郁肺证效果显著,能够有效地改善患儿的临床症状和肺功能,并且用药安全性好,能够促进患儿康复。
Objective To observe the effects of Modified Maxing Shigan decoction (麻杏石甘汤) on symptom score and pulmonary function in children with viral pneumonia with wind-heat obstructing lung syndrome, and provide reference for clinical diagnosis and treatment. Methods A hundred cases of children with viral Pneumonia with wind-heat obstructing lung syndrome in our hospital from October 2018 to September 2019 were selected, and they were divided into two groups according to the random number table method. Fifty cases in the control group were treated with conventional western medicine and those in the observation group were treated with Modified Maxing Shigan decoction on the basis of control group. The total effective rate, clinical symptom score, clinical symptom disappearance time, pulmonary function and adverse reactions of two groups were compared. Results The total effective rate in the observation group was significantly higher than that in the control group (P<0.05). There was no significant difference in fever, cough, wheezing or lung snoring scores between two groups before treatment (P>0.05). After treatment, the scores of each symptom in two groups were significantly reduced, and the observation group’s score was significantly lower than that of the control group (P<0.05). The fever, cough, wheezing disappearance time and hospital stay in the observation group were significantly shorter than those in the control group (P<0.05), while the lung snoring sounds were slightly shorter than those in the control group, and the differences were not statistically significant (P>0.05). There were no significant differences in forced vital capacity (FVC), forced expiratory volume (FEV1) or maximum expiratory flow (PEF) in two groups before treatment (P>0.05). All the items were significantly improved after treatment, and the improvement in the observation group was better than that in the control group (P<0.05). No serious adverse reactions occurred in two groups. Conclusion Modified Maxing Shigan decoction has significant effect on children with viral pneumonia with wind-heat obstructing lung syndrome. It can effectively improve the clinical symptoms and lung function of patients, and the safety of medication is good, and it can promote the rehabilitation of children.
