桂枝温肺化痰汤对呼吸道感染后咳嗽及气道炎症和免疫功能的影响

Effect of Guizhi Wenfei Huatan Decoction (桂枝温肺化痰汤) on Airway Inflammation and Immune Function in Patients with Cough after Respiratory Tract Infection

目的观察桂枝温肺化痰汤对呼吸道感染后咳嗽气道炎症和免疫功能的影响。方法将2016年12月—2019年12月医院收治的呼吸道感染后咳嗽患者86例分为观察组和对照组,每组43例。对照组口服复方甲氧那明胶囊,2片/次,3次/d。观察组在对照组的基础上,口服桂枝温肺化痰汤治疗。两组均治疗2周。比较两组咳嗽消失时间、咳痰消失时间和气急消失时间;比较痰标本中炎症介质TNF-α、SP、IL-8和sIgA水平;比较治疗前后CD^(+)_(3)、CD^(+)_(4)、CD^(+)_(8)和CD^(+)_(4)/CD^(+)_(8)水平和治疗疗效。结果研究组治疗总有效率为90.70%(39/43)高于对照组治疗总有效率的74.42%(32/43)(P<0.05);观察组治疗后咳嗽、咳痰、咽痒和哮鸣音等中医证候积分低于对照组(P<0.05);观察组患者临床症状消失时间均短于对照组(P<0.05);观察组治疗后痰标本中炎症介质较对照组有了更大幅度的降低,sIgA水平较对照组有了更大幅度的提升(P<0.05);观察组治疗后免疫功能较对照组有了更明显的改善(P<0.05)。结论桂枝温肺化痰汤结合西药常规疗法可有效的改善呼吸道感染后患者的临床症状,缓解病情、缩短病程,其机制可能与减轻气道炎症反应以及提高机体免疫力有关,值得临床进一步研究探讨。

Objective To observe the effect of Guizhi Wenfei Huatan Decoction(桂枝温肺化痰汤,GWHD)on airway inflammation and immune function in patients with cough after respiratory tract infection.Methods Eighty-six patients with cough after respiratory infection admitted to our hospital from December 2016 to December 2019 were divided into observation group and control group,43 cases in each group.The patients in the control group took Compound Methoxyphenamine Capsules,2 tablets/time,3 times/day.The observation group was treated with GWHD on the basis of the control group.Both groups were treated for 2 weeks.Before and after treatment,the changes of symptom score and therapeutic effect of the two groups were compared.The disappearance time of cough,expectoration and shortness of breath were compared.The levels of inflammatory mediators TNF-α,SP,IL-8 and sIgA in sputum samples were compared.The levels of CD^(+)_(3),CD^(+)_(4),CD^(+)_(8)and CD^(+)_(4)/CD^(+)_(8)before and after treatment were compared.Results The scores of cough,sputum,itch and wheezing were lower in the observation group than those in the control group(P<0.05).The disappearance time of cough,sputum and shortness of breath in the observation group were shorter than those in the control group(P<0.05).After treatment,the inflammatory mediators in sputum samples of the observation group were significantly lower than that of the control group,and the SIgA level was significantly higher than that of the control group(P<0.05).The immune function of the observation group was significantly improved than that of the control group(P<0.05).The total effective rate of the study group was higher than that of the control group(P<0.05).Conclusion GWHD combined with western medicine conventional therapy can effectively improve the clinical symptoms of patients with respiratory tract infection,alleviate the condition and shorten the course of disease.Its mechanism may be related to reducing airway inflammation and improving the body immunity,worthy of further clinical study.
