身痛逐瘀汤联合俯卧位顺势牵引治疗腰椎间盘突出症的临床应用研究

Clinical Application of Shentong Zhuyu Decoction(身痛逐瘀汤)Combined with Prone Position Homeopathy Traction in Treatment of Lumbar Disc Herniation

目的探讨身痛逐瘀汤联合俯卧位顺势牵引治疗腰椎间盘突出症在临床应用方面的影响。方法选择医院2018年3月—2019年3月收治的200例腰椎间盘突出症患者,随机数字法分为对照组和治疗组,每组100例,对照组给予俯卧位顺势牵引治疗,治疗组在对照组的基础上联合身痛逐瘀汤进行治疗,观察两组患者在治疗过程中的临床疗效、视觉模拟标尺法(VAS)评分、JOA评分、血清炎性因子水平及治疗前后患者生活质量评分。结果对照组临床总有效率为79.00%(79/100),治疗组临床总有效率为90.00%(90/100)(P<0.05);治疗后,两组患者VAS评分均显著降低(P<0.05),治疗组明显低于对照组(P<0.05);JOA评分均显著升高(P<0.05),治疗组明显高于对照组(P<0.05);两组患者CRP、IL-6、IL-1β、TNF-α水平均显著降低(P<0.05),治疗组明显低于对照组(P<0.05);治疗组患者精神状态、躯体活动、社会功能评分均高于对照组(P<0.05)。结论身痛逐瘀汤联合俯卧位顺势牵引治疗腰椎间盘突出症疗效确切,有效减轻患者痛苦,提高患者生活质量,在研究过程中,两组患者均无严重不良反应,安全有效,值得在临床上推广应用。

Objective To investigate the clinical application of Shentong Zhuyu Decoction(身痛逐瘀汤)combined with prone position homeopathy traction in the treatment of lumbar disc herniation.Methods A total of 200 patients with lumbar disc herniation who were admitted to our hospital from March 2018 to March 2019 were randomly divided into the control group and the treatment group,100 cases in each group.The control group was given prone position homeopathy traction.In the treatment group,Shentong Zhuyu Decoction was used on the basis of the control group.The clinical efficacy,visual analog scale method(VAS)score,JOA score,serum inflammatory factor level,the serum inflammation factors and quality of life scores were observed before and after treatment.Results The total effective rate was 79.00%(79/100)in the control group and 90.00%(90/100)in the treatment group(P<0.05).After treatment,the VAS scores of the two groups were significantly lower(P<0.05),and the treatment group’s VAS score was significantly lower than that of the control group(P<0.05).JOA scores were significantly increased(P<0.05),and the treatment group’s JOA score was significantly higher than that of the control group(P<0.05).Levels of CRP,IL-6,IL-1βand TNF-αwere significantly decreased in both groups(P<0.05).And those of the treatment group were obviously lower than those in the control group(P<0.05).The mental state,physical activity and social function scores of the treatment group were higher than those of the control group(P<0.05).Conclusion The treatment of lumbar intervertebral disc herniation with the combination of Shentong Zhuyu Decoction combined with prone position homeopathy traction is effective,which can effectively alleviate the suffering and improve the quality of life of patients.During the research,there is no serious adverse reaction in both groups.It is safe and effective,worthy of clinical application.
