聚焦解决模式联合助产士门诊服务对高危妊娠孕妇分娩结局及心理状态的影响

The effect of focus solution mode combined with midwife clinic service on the outcome and mental state of pregnant women with high risk pregnancy

目的探讨聚焦解决模式联合助产士门诊服务对高危妊娠孕妇分娩结局及心理状态的影响。方法选取2019年6月至2020年8月于广州市花都区妇幼保健院(胡忠医院)进行孕期保健的高危孕妇460例,按照简单数字法随机分成对照组和观察组各230例。对照组孕妇予以实施常规孕期护理管理,观察组孕妇在常规护理管理的基础上联合予以助产士门诊服务及聚焦解决模式心理干预。比较干预前后两组孕妇的分娩结局、心理状态及护理满意度。结果①观察组的剖宫产率、产后并发症发生率均低于对照组,差异有统计学意义(均P<0.05);②观察组新生儿窒息、巨大儿及早产儿发生率低于对照组,而新生儿出生1 min Apgar评分显著高于对照组,差异均有统计学意义(均P<0.05);③观察组产妇的抑郁自评量表(SDS)、焦虑自评量表(SAS)评分均低于对照组,差异有统计学意义(均P<0.05);④观察组的护理满意度显著高于对照组,差异有统计学意义(P<0.05)。结论采用聚焦解决模式心理干预联合助产士门诊服务对高危妊娠孕妇进行产前护理管理,能有效改善孕妇心理状态,改善分娩结局,护理效果较好。

Objective To explore the effect of focused solution model combined with midwife clinic service on the delivery outcome and psychological status of pregnant women with high-risk pregnancies.Methods A total of 460 high-risk pregnant women who received pregnancy health care in Huadu District Maternal and Child Health Hospital(Huzhong Hospital)of Guangzhou from June 2019 to August 2020 were selected and randomly divided into control group and observation group with 230 cases in each group according to simple number method.The pregnant women in the control group were given routine nursing management during pregnancy,while the pregnant women in the observation group were given midwife clinicservice and focused solution mode psychological intervention on the basis of routine nursing management.The delivery outcome,psychological state and nursing satisfaction of the two groups were compared before and after the intervention.Results①The rates of cesarean section and postpartum complications in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).②The incidence of neonatal asphyxia,macrosomia and premature infants in the observation group was lower than that in the control group,while the Apgar score of neonates 1 minute after birth was significantly higher than that in the control group,the differences were statistically significant(P<0.05).③The scores of Self-Rating Depression Scale(SDS)and Self-Rating Anxiety Scale(SAS)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).④The nursing satisfaction of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusions Adoption of focused solution mode of psychological intervention combined with midwife clinic service to prenatal care management of high-risk pregnant women can effectively improve the psychological state of pregnant women,improve the outcome of childbirth,and the nursing effect is good.
