宣白承气汤加减治疗重症流行性感冒(毒热壅肺证)临床研究

Clinical Observation on Modified Xuanbai Chengqi Decoction in treatment of severe influenza(toxic-heat obstructing lung syndrome)

目的观察宣白承气汤加减治疗重症流行性感冒(毒热壅肺证)的临床效果。方法将我院就诊的重症流感患者84例采用随机数字表法分为对照组与观察组各42例。对照组给予西药综合治疗,观察组在对照组基础上给予宣白承气汤加减治疗。比较两组治疗后的临床疗效、主要症状消失时间,治疗前后的中医证候积分、血清超敏C-反应蛋白(hs-CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、肌酸激酶同工酶(CK-MB)水平和淋巴细胞数;以及治疗期间两组患者的不良反应总发生率。结果治疗后,观察组的治疗总有效率明显高于对照组(P<0.05);观察组完全退热时间、咳嗽消失时间、咽喉肿痛消失时间和周身酸痛消失时间均低于对照组(P<0.05)且TNF-α、IL-6、hs-CRP水平及中医证候积分均低于对照组(P<0.05);但两组CK-MB水平、淋巴细胞数和治疗期间不良反应总发生率比较无明显差异(P>0.05)。结论宣白承气汤加减治疗重症流行性感冒(毒热壅肺证)临床疗效显著,可有效改善患者临床症状,抑制机体炎症反应,且安全性可靠。

Objective To observe the clinical effect of Modified Xuanbai Chengqi Decoction in treatment of severe influenza(toxic-heat obstructing lung syndrome).Methods A total of 84 patients with severe influenza were randomly divided into control group and observation group with 42 cases each.Control group was given comprehensive treatment with western medicine,while observation group was treated with Modified Xuanbai Chengqi Decoction on the basis of control group.The clinical efficacy,disappearance time of main symptoms,scores of TCM syndromes,levels of serum hpyersensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),creatine kinase isoenzyme CK-MB,the lymphocytes count before and after treatment,and the total incidence of adverse reactions during treatment were compared between the two groups.Results After treatment,the total response rate of treatment in observation group was significantly higher than that in control group(P<0.05),the complete antipyretic time disappearance time of cough,sore throat and body aches in observation group were all shorter than those in control group(P<0.05),the levels of TNF-α,IL-6 and hs-CRP,and the scores of TCM syndromes were lower than those in control group(P<0.05),there were no significant differences in CK-MB level lymphocytes count and total incidence of adverse reactions during treatment between the two groups(P>0.05).Conclusion The clinical curative effect of Modified Xuanbai Chengqi Decoction is significant on severe influenza(toxic-heat obstructing lung syndrome).It can effectively improve clinical symptoms of patients,and inhibit inflammatory response,which is safe and reliable.
