川芎注射液联合推拿、运动疗法对椎动脉型颈椎病患者临床症状及椎基底动脉结构、功能的影响

Effect of Chuanxiong Injection combined with massage and exercise on clinical symptoms and vertebrobasilar artery structure,function in patients with vertebral artery type cervical spondylosis

目的观察川芎注射液联合推拿、运动疗法对椎动脉型颈椎病(CSA)患者临床症状和椎基底动脉结构、功能的影响。方法将我院118例CSA患者随机分为对照组与观察组各59例。对照组采用推拿与运动疗法治疗,观察组在对照组治疗基础上联合采用川芎注射液治疗。比较两组患者治疗临床疗效、治疗前后中医证候积分、椎基底动脉结构和功能、血液流变学情况。结果观察组总有效率高于对照组(P<0.05);治疗后,两组中医证候积分、经颅多普勒超声(TCD)异常率、双侧椎动脉和基底动脉血流速度均下降(P<0.05);且观察组均低于对照组(P<0.05);两组高切、低切全血粘度,血浆粘度,红细胞聚集指数、变形指数和纤维蛋白原均下降(P<0.05),且观察组均低于对照组(P<0.05)。结论川芎注射液联合按摩、运动疗法治疗CSA患者疗效显著,能明显改善临床症状,促进椎基底动脉的结构和功能的恢复,降低血液的黏稠度。

Objective To observe the effects of Chuanxiong Injection combined with massage and exercise on the clinical symptoms and the structure and function of the vertebral basilar artery in patients with cervical spondylosis of vertebral artery type(CSA). Methods 118 CSA patients were randomly divided into control group and observation group with 59 cases each.Control group was treated with massage and exercise therapy,and observation group was treated with Chuanxiong Injection on the basis of control group. The clinical efficacy,TCM syndrome scores before and after treatment,the structure and function of vertebrobasilar artery,hemorheology were compared between the two groups. Results The total effective rate of observation group was higher than control group(P<0.05);after treatment,TCM syndrome score,Transcranial Doppler ultrasound(TCD)abnormal rate,bilateral vertebral artery and basilar artery blood flow velocity of both groups decreased(P<0.05),observation group were all lower than control group(P<0.05);high-cut and low-cut whole blood viscosity,plasma viscosity,erythrocyte aggregation index,deformation index,and fibrinogen of both groups decreased,observation group were all lower than control group(P<0.05). Conclusion Chuanxiong Injection combined with massage and exercise therapy is effective in treatment of CSA,which can obviously improve clinical symptoms,promote the restoration of the structure and function of vertebrobasilar artery and reduce blood viscosity.
