不同糖代谢人群的舌象客观化特征研究

Objective Characteristics of Tongue Image in Different Glucose Metabolism Populations

目的探讨不同糖代谢状态人群的舌象客观化特征。方法纳入2型糖尿病患者278例(DM组),糖尿病前期人群296例(IGR组),糖代谢正常人群300例(NOR组),采集糖化血红蛋白(HbA1c)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、空腹游离脂肪酸(FFA)、餐后2 h游离脂肪酸(2 h FFA)及胰岛素抵抗指数(HOMA-IR)。采用TDA-1型小型舌像仪采集患者的舌象参数,包括舌质颜色指标(zhiR、zhiG、zhiB、zhiH、zhiI、zhiS、zhiL、zhia、zhib)及舌苔颜色参数(taiR、taiG、taiB、taiH、taiI、taiS、taiL、taila、taib)。结果生化指标方面,与NOR组相比,DM组与IGR组Homa-IR升高(P<0.05,P<0.01),DM组TG、FFA、2 hFFA升高,HDL降低(P<0.01)。IGR组FFA升高(P<0.05)。与IGR组相比,DM组TG、2 hFFA升高,HDL降低(P<0.01)。舌质方面,与NOR组、IGR组相比,DM组zhiB、zhiG、zhiH、zhiI、zhiL降低(P<0.05,P<0.01)。舌苔方面,与NOR组、IGR组相比,DM组taiB、taiR、taiG、taiH、taiI、taiL降低(P<0.01),taiS、taia、taib升高(P<0.05)。结论不同糖代谢状态人群的舌象客观化指标存在显著差异。舌色可能受糖代谢水平影响,舌象客观化指标在一定程度上能反映糖代谢水平。

Objective To explore the objective features of tongue image in people with different glucose metabolism.Methods A total of 278 patients with type 2 diabetes(DM group),296 patients with pre-diabetes(IGR group),300 patients with normal glucose metabolism(NOR group)were included,and glycated hemoglobin(HbA1c),triglyceride(TG)and total cholesterol(TC),low-density lipoprotein(LDL),high-density lipoprotein(HDL),fasting free fatty acid(FFA),2 h free fatty acid(2 h FFA)and insulin resistance index(HOMA-IR)were collected.Using the TDA-1 small tongue camera,the tongue image parameters of patients was collected.Tongue image indicators included tongue color indicators(zhiR,zhiG,zhiB,zhiH,zhiI,zhiS,zhiL,zhia,zhib)and tongue coating color parameter(taiR,taiG,taiB,taiH,taiI,taiS,taiL,taia,taib).Results In terms of biochemical indicators,compared with the NOR group,Homa-IR increased in the DM group and the IGR group(P<0.05,P<0.01),and TG,FFA and 2 hFFA increased in the DM group,and HDL decreased(P<0.01).FFA increased in the IGR group(P<0.05).Compared with the IGR group,TG and 2 h FFA increased and HDL decreased in the DM group(P<0.01).In terms of tongue color,compared with the NOR group and the IGR group,zhiB,zhiG,zhiH,zhiI and zhiL in the DM group were reduced(P<0.05,P<0.01).In terms of tongue coating color,compared with the NOR group and the IGR group,taiB,taiR,taiG,taiH,taiI and taiL in the DM group were decreased(P<0.01),and taiS,taia and taib were increased(P<0.05).Conclusion There are significant differences in the objective indicators of tongue image in people with different glucose metabolism.The color of the tongue may be affected by the level of glucose metabolism,and the objective indicators of the tongue image can reflect the level of glucose metabolism to a certain extent.
