健脾祛湿汤加二甲双胍治疗糖尿病的效果评估分析

Effect of Jianpi Qushi Decoction(健脾祛湿汤)and Metformin in Treatment of Diabetes Mellitus

目的探讨健脾祛湿汤加二甲双胍治疗糖尿病的效果评估分析。方法将2019年1月—2020年1月在该院内分泌科治疗的108例糖尿病患者随机分为两组,对照组54例单用二甲双胍治疗,在此基础上,观察组54例使用健脾祛湿汤治疗,对比两组的临床疗效、血糖及胰岛功能指标变化、血脂指标变化、中医证候积分变化。结果观察组治疗有效率为94.44%(51/54),明显高于对照组的74.07%(40/54)(P<0.05);两组治疗前空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbAlc)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)差异无统计学意义(P>0.05),观察组治疗后FPG、2 hPG、HbAlc、FINS、HOMA-IR均明显低于对照组(P<0.05);两组治疗前后总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平差异无统计学意义(P>0.05),观察组治疗后TC、TG、LDL-C水平明显低于对照组,而HDL-C水平明显高于对照组(P<0.05);两组治疗前多食易饥、口渴喜饮、倦怠乏力、小便频多、头身困重、肢体麻木、气短懒言等中医证候积分差异无统计学意义(P>0.05),观察组治疗后多食易饥、口渴喜饮、倦怠乏力、小便频多、头身困重、肢体麻木、气短懒言等中医证候积分明显低于对照组(P<0.05)。结论健脾祛湿汤加二甲双胍治疗糖尿病的效果显著,能有效降低糖血糖、血脂,减轻机体胰岛素抵抗,保护胰岛功能,促进中医证候的缓解,具有积极的临床意义。

Objective To explore the effect of Jianpi Qushi Decoction(健脾祛湿汤)and metformin in the treatment of diabetes.Methods A total of 108 diabetic patients who were treated in the department of endocrinology in the hospital from January 2019 to January 2020 were randomly divided into two groups.Fifty-four cases in the control group were treated with metformin alone.On this basis,54 cases in the observation group were treated with Jianpi Qushi Decoction.The clinical efficacy,changes in blood glucose and islet function indexes,changes in blood lipid indexes,and changes in TCM syndrome scores were compared between the two groups.Results The effective rate of the observation group was 94.44%(51/54),which was significantly higher than that of the control group(74.07%,40/54)(P<0.05).There was no significant difference in FPG,2 hPG,HbAlc,FINS or HOMA-IR between the two groups before and after treatment(P>0.05).The levels of FPG,2 hPG,HbAlc,FINS and HOMA-IR in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in TC,TG,HDL-C or LDL-C between the two groups before and after treatment(P>0.05).After treatment,levels of TC,TG and LDL-C in the observation group were significantly lower than those in the control group,while HDL-C levels were significantly higher than those in the control group(P<0.05).The scores of TCM syndromes such as frequent urination,heavy head and body sleepiness,numbness of limbs,shortness of breath and laziness were significantly lower than those of the control group(P<0.05).Conclusion Jianpi Qushi Decoction combined with metformin has a significant effect on diabetes mellitus.It can effectively reduce blood glucose and blood lipid,reduce insulin resistance,protect the function of islet,and promote the remission of TCM syndrome,which has a positive clinical significance.
