针灸治疗慢性功能性便秘的安全性及有效性评价

Safety and Effectiveness Evaluation of Acupuncture for Chronic Functional Constipation

目的分析针灸治疗慢性功能性便秘(CFC)的安全性及有效性,为临床治疗提供指导。方法选取该院(2016年1月—2018年12月)收治的CFC患者130例,根据治疗方法分为两组。对照组给予常规对症治疗,观察组在对照组基础上给予针灸治疗,分析两组患者治疗后的疗效、不良反应及胃肠功能、便秘症状、胃动素(MTL)、血管活性肠肽(VIP)、P物质(SP)、一氧化氮(NO)的变化。结果观察组总有效率(90.77%,57/65)高于对照组(76.92%,50/65),差异具有统计学意义(P<0.05)。两组治疗前全胃肠排出率、胃排空率、排便困难、大便性状、排便时间及排便频率组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后全胃肠排出率、胃排空率、排便频率高于对照组,排便困难、大便性状、排便时间低于对照组(P<0.05)。两组治疗前相关因子水平组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后MTL、VIP、SP水平高于对照组,NO水平低于对照组(P<0.05)。结论针灸治疗CFC可调节MTL、VIP、SP、NO的表达水平,改善胃肠功能,缓解便秘症状。

Objective To analyze the safety and effectiveness of acupuncture and moxibustion in the treatment of chronic functional constipation(CFC)and provide guidance for clinical treatment.Methods From January 2016 to December 2018,130 patients with CFC were divided into two groups according to the treatment method.The control group was treated with routine symptomatic treatment,and the observation group was treated with acupuncture and moxibustion on the basis of the control group.The curative effect,adverse reactions and gastrointestinal function,constipation symptoms and the changes of motilin(MTL),vasoactive intestinal peptide(VIP),P(SP),nitric oxide(NO)in the two groups were analyzed.Results The total effective rate(90.77%,59/65)in the observation group was significantly higher than that in the control group(76.92%,50/65)(P<0.05).There was no significant difference in total gastrointestinal excretion rate,gastric emptying rate,defecation difficulty,defecation traits,defecation time and defecation frequency between the two groups before treatment(P>0.05).After treatment,the total gastrointestinal excretion rate,gastric emptying rate and defecation frequency in the observation group were higher than those in the control group.Thedifficult defecation,defecation traits and defecation time were lower than those in the control group(P<0.05).There was no significant difference in the level of related factors between the two groups before treatment(P>0.05).After treatment,the levels of MTL,VIP and SP in the observation group was higher than those in the control group,and the level of NO was lower than that in the control group(P<0.05).Conclusion Acupuncture and moxibustion treatment of CFC can regulate the expressions of MTL,VIP,SP and NO,improve gastrointestinal function and relieve constipation symptoms.
