灸法干预缺血性脑卒中患者肢体功能障碍的选穴规律研究

Study on the Rule of Moxibustion in Intervention of Limb Dysfunction in Patients with Ischemic Stroke

目的:运用数据挖掘技术分析灸法在缺血性脑卒中患者肢体功能障碍康复中应用的选穴特点,规范灸法干预。方法:检索CNKI、万方数据、维普网、中国生物医学文献数据库、PubMed数据库中有关灸法干预缺血性脑卒中患者肢体功能障碍的文献,检索时间为建库至2020年5月。应用频数分析、聚类分析和关联规则分析进行数据挖掘。结果:共纳入灸法干预缺血性脑卒中患者肢体功能障碍的统计源核心期刊论文68篇,共涉及119个穴位,腧穴的选择上以局部取穴为主,运用频数最高的穴位依次是曲池、合谷、肩髃、外关。阳经腧穴特别是手阳明大肠经使用频数最多。在特定穴中以交会穴使用频数最多,共产生203条关联规则,曲池、合谷、肩髃、外关、手三里等穴之间具有强关联性。聚类分析结果提示灸法干预缺血性脑卒中肢体障碍的选穴可分为以臂臑、肩髎、肩前、肩贞等穴为主和以曲池、合谷、肩髃、外关、手三里等穴为主的两大类。结论:艾灸干预缺血性脑卒中肢体功能障碍常用穴位是曲池、合谷、肩髃、外关、手三里,在经络选择上重视阳经腧穴,同时注意交会穴的使用。

Objective:To analyze the characteristics of acupoint selection of moxibustion in limb dysfunction rehabilitation nursing of patients with ischemic stroke by using data mining technology,and to standardize the intervention of moxibustion.Methods:Articles about moxibustion intervention on limb dysfunction in ischemic stroke were searched in CNKI,Wanfang data,VIP,China Biomedical Literature Database and Pubmed database from the establishment of the database to May 2020.Data mining is carried out by using frequency analysis,cluster analysis and association rule analysis.Results:A total of 68 statistical core journals of moxibustion intervention on limb dysfunction in ischemic stroke were included.A total of 119 acupoints were collected,and local acupoints were mainly selected in the selection of acupoints.Quchi(LI11),Hegu(LI4),Jianyu(LI15)and Waiguan(SJ5)were used most frequently.Yang meridian acupoints,especially hand Yangming large intestine meridian,are most frequently used.Among the specific acupoints,the intersection acupoint is the most frequently used,resulting in a total of 203 association rules.Quchi(LI11),Hegu(LI4),Jianyu(LI15),Waiguan(SJ5),Shousanli(LI10)and other acupoints have a strong correlation.The results of cluster analysis suggest that the acupoints of moxibustion intervention on limb disorders of ischemic stroke can be divided into two categories:Binao(LI14),Jianliao(SJ14),Jianqian and Jianzhen(SI9),and Quchi(LI11),Hegu(LI4),Jianyu(LI15),Waiguan(SJ5)and Shousanli(LI10).Conclusion:The common acupoints of moxibustion in intervention of limb dysfunction in ischemic stroke are Quchi(LI11),Hegu(LI4),Jianyu(LI15),Waiguan(SJ5)and Shousanli(LI10).We should pay attention to Yang meridian acupoints and the use of intersection acupoints in the selection of meridians and collaterals.
