益母草注射液联合缩宫素对剖宫产产后出血的预防效果及对PT、FIB水平的影响

The Preventive Effect of Leonurus Injection Combined with Oxytocin on Postpartum Hemorrhage after Cesarean Section and Its Effect on PT and FIB Levels

目的研究益母草注射液联合缩宫素对剖宫产产后出血的预防效果及对血浆凝血酶原时间(PT)、纤维蛋白原(FIB)的影响。方法选取2017年12月—2019年6月收治的62例剖宫产产妇,按照随机数字表法分为对照组(31例)和观察组(31例)。对照组给予缩宫素治疗,观察组在对照组的基础上给予益母草注射液。比较2组术后24 h临床疗效、不良反应发生率,术中及术后2 h、24 h出血量,术前和术后24 h凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、PT、FIB水平。结果与对照组相比,观察组的总有效率为90.32%,高于对照组的77.42%(P<0.05);与对照组相比,观察组术中、术后2 h和术后24 h出血量均低于对照组(P>0.05);与术前相比,术后24 h 2组TT、APTT、PT均缩短,且观察组短于对照组;与术前相比,术后24 h 2组FIB水平均升高,且观察组高于对照组(P<0.05);与对照组相比,观察组不良反应发生率为19.36%,低于对照组的48.39%(P<0.05)。结论益母草注射液联合缩宫素可有效预防剖宫产产后出血,减少术中及术后出血量,缩短TT、APTT、PT,改善FIB水平,从而改善凝血功能,且疗效显著,不良反应少。

Objective To study the preventive effect of leonurus injection combined with oxytocin on postpartum hemorrhage after cesarean section and the effect on plasma prothrombin time and fibrinogen.Methods From December 2017 to June 2019,62 cases of cesarean section women in our hospital were selected as the study objects.According to the random number table method,they were divided into control group(n=31)and observation group(n=31).The control group was treated with oxytocin,and the observation group was treated with leonurus injection on the basis of the control group.The clinical efficacy,adverse reactions,bleeding volume,PTT,FIB was compared between the two groups.Results Compared with the control group,the total effective rate of the observation group was 90.32%,which was higher than that of the control group(77.42%)(P<0.05).Compared with the control group,the amount of bleeding in the observation group was lower than that in the control group(P<0.05).Compared with the preoperative time,the TT,APTT and PT of the 24 h two groups shorten,and the observation group was lower than the control group.Compared with the control group,the FIB level of the 24 h two groups increased.And the observation group was higher than the control group(P<0.05).The incidence of adverse reactions in the observation group was 19.36%,which was lower than that in the control group(48.39%)(P<0.05).Conclusion Leonurus injection combined with oxytocin can effectively prevent postpartum hemorrhage during cesarean section,reduce intraoperative and postoperative bleeding,shorten TT,APTT,PT improve FIB level,so as to improve coagulation function,with significant curative effect and less adverse reactions.
