摘要
目的分析玻璃体切除联合眼内异物摘出手术术后视力影响的因素。方法回顾性分析信阳市中心医院2013年2月至2019年2月眼外伤所致眼内异物者60例(60眼)的临床资料。所有患者行玻璃体切除联合眼内异物摘出手术,分析影响手术效果及术后视力的因素。结果手术成功率为93.33%(56/60);术后视力<0.1的独立影响因素为视网膜脱离[OR=0.054,95%CI(0.013~0.223),P<0.05];异物较大[OR=0.053,95%CI(0.006~0.451),P<0.05]及晶状体损伤[OR=0.274,95%CI(0.093~0.807),P<0.05];术后有1例增生性玻璃体视网膜病变、1例牵引性视网膜脱离、眼内炎及黄斑前膜形成各1例;患者术前术后的视力差异比较有统计学意义(P<0.05)。结论玻璃体切除眼内异物摘出联合手术的总有效率较高,术后视力的影响因素主要是视网膜脱离、异物较大及晶状体损伤。
Objective To analyze the influential factors on visual acuity of surgery of combined vitrectomy with intraocular foreign body extraction.Methods The clinical data of 60 eyes of 60 patients with intraocular foreign body due to ocular trauma who received combined surgery of vitrectomy and intraocular foreign body extraction from Feb.2013 to Feb.2019 in Xinyang Central Hospital were retrospectively analyzed.The influencing factors of surgical efficacy and visual acuity prognosis were analyzed.Results The success rate of extraction was 93.33%(56/60).The independent influence factor of visual acuity<0.1 was retinal detachment[OR=0.054,95%CI(0.013~0.223),P<0.05,larger foreign bodies[OR=0.053,95%CI(0.006~0.451),P<0.05]and lens injury[OR=0.274,95%CI(0.093~0.807),P<0.05].After operation,there were 1 case of proliferative vitreoretinopathy,1 case of tractional retinal detachment,1 case of intraocular inflammation and 1 case of macular epiretinal membrane.The difference of visual acuity was statistically significant before and after surgery(P<0.05).Conclusion The success rate of vitrectomy combined with extraction of intraocular foreign bodies is higher,and the main factors effecting the postoperative vision are retinal detachment,foreign body size and lens injury.
