硬核白内障小切口手法白内障手术与超声乳化术的效果比较

Comprison of the efficacy between small incision manual cataract surgery and phacoemulsification in patients with hard nuclear cataract

目的比较硬核白内障行小切口手法娩核白内障摘出手术与超声乳化术的临床效果。方法回顾性研究。选取南阳南石医院2017年3月至2019年3月收治的112例(112眼)硬核白内障为研究对象。患者根据手术方式分为两组:手法组56例(56眼)行小切口手法娩核白内障手术;超乳组56例(56眼)行超声乳化手术。术后均随访3个月,比较两组术后视力、角膜散光及并发症。结果术后视力(UCVA,logMAR)手法组高于超乳组,差异具有统计学意义(F时点=867.283,F_(交互)=34.691,F_(组间)=17.833;均P<0.05)。角膜散光程度手法组患者均低于超乳组,差异具有统计学意义(F时点=24.773,F_(交互)=18.451,F_(组间)=16.037;均P<0.05)。并发症发生率手法组为8.93%(5/56)低于超乳组的25.00%(14/56),差异有统计学意义(χ^(2)=5.134,P=0.023)。结论小切口手法娩核治疗硬核白内障术后角膜反应较轻,视力恢复较快,角膜散光轻且并发症发生率低。

Objective To compare the clinical efficacy of small incision non-phacoemulsification manual muclear delivery and phacoemulsification for the treatment of hard nuclear cataract.Methods This was a retrospective study.A total of 112 eyes of 112 of cases of hard nuclear cataract from Mar.2017 to Mar.2019 in Nanyang Nanshi Hospital were collected.The patients were divided into two groups based on the surgrical methods.The manual group with 56 eyes of 56 cases received small incision non-phacoemulsification manual nuclear extraction,and the phaco group with 56 eyes of 56 cases received phacoemulsification.All patients were followed up for 3 months to compare the postoperative visual acuity,corneal astigmatism and incidence of complications between the two groups.Results Postoperative uncorrected visual acuity(UCVA,LogMAR)in the manual group was significantly higher than that in the phaco group,the difference was statistically significant(F_(time point)=867.283,F_(interaction)=34.691,Fbetween groups=17.833;all P<0.05).The degree of postoperative corneal astigmatism of the manual group was lower than that of the phaco group,the difference was statistically significant(F_(time point)=24.773,F_(interaction)=18.451,Fbetween groups=16.037;all P<0.05).The incidence rate of manual group was 8.93%(5/56),which was lower than 25.00%(14/56)of phaco group,the difference was statistically significant(χ^(2)=5.134,P=0.023).Conclusion The corneal reaction after small incision manual cataract surgery for hard nuclear cataract is relatively light,the visual acuity recovery is quickly,the degree of corneal astigmatism is light.The incidence of complications is low.
