目的:分析西药房处方调剂差错、处方不合理等问题,制定预防措施,尽可能降低处方错误的出现,进而提高西药房工作质量。方法:随机选取上饶市立医院2018年1月10日至2020年2月1日住院部西药房处方调剂差错200例行观察分析,总结出现处方调剂差错的原因,并根据原因制定相应的处方管理措施。结果:200例处方差错包括规格及剂量错误(41.50%)、用法用量不当(32.50%)、用药不当(21.00%)、触发用药禁忌(5.00%);患者不按照医嘱用药(19.00%)、药品包装相似(25.00%)、药品同种规格不同(18.00%)、药品名称相似(19.50%)、药师对患者了解不足(9.00%)、医师对患者了解不足(9.50%)。结论:西药房处方调剂差错主要原因包括患者药物使用认知不足、药师与医师合作不够紧密、药师对药物了解不够深入、药物使用管理力度不足等,因此在管理上可以采取强化患者药物使用认知、加强药师培训、强化药物使用管理、协调药师与医师关系等措施,以有效规避处方调剂出现差错,提高西药房处方管理质量。

Objective:To analyze the problems of prescription dispensing errors and unreasonable prescriptions in western pharmacies,and formulate preventive measures to minimize the occurrence of prescription errors and improve the quality of western pharmacies.Methods:Randomly selected 200 cases of prescription dispensing errors in the western pharmacy of Shangrao Municipal Hospital from January 10,2018 to February 1,2020.Observation and analysis were conducted to summarize the causes of prescription dispensing errors and formulate corresponding prescription management measures according to the reasons.Results:200 cases of prescription errors included specification and dosage errors(41.50%),improper usage and dosage(32.50%),improper medication(21.00%),trigger medication contraindications(5.00%);patients not taking medication as prescribed by doctors(19.00%),and drug packaging Similar(25.00%),different specifications of the same drugs(18.00%),similar names of drugs(19.50%),pharmacists do not know enough about patients(9.00%),physicians do not know enough about patients(9.50%).Conclusion:The main reasons for prescription adjustment errors in western pharmacies include insufficient knowledge of patients'drug use,insufficient cooperation between pharmacists and physicians,insufficient understanding of drugs by pharmacists,and insufficient management of drug use.Therefore,management can be adopted to strengthen patients'knowledge of drug use,Measures such as strengthening the training of pharmacists,strengthening drug use management,and coordinating the relationship between pharmacists and physicians,in order to effectively avoid errors in prescription dispensing and improve the quality of prescription management in western pharmacies.
