自拟糖肾方治疗糖尿病肾病的临床疗效观察

Clinical efficacy of self-formulated Tangshen recipe in the treatment of diabetic nephropathy

目的研究自拟糖肾方治疗糖尿病肾病(DN)的疗效。方法84例糖尿病肾病患者,采用随机数字表法分为糖肾方组与常规组,各42例。常规组患者采用基础治疗,糖肾方组患者在基础治疗上加用自拟糖肾方治疗。比较两组患者治疗前后糖脂代谢指标[空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)],治疗前后尿白蛋白指标(24 h尿微量白蛋白排泄率、24 h尿蛋白定量)。结果治疗后,两组患者的FBG、2 h PG、HbA1c、TC、TG、LDL-C水平均低于本组治疗前,HDL-C水平均高于本组治疗前,且糖肾方组FBG、2 h PG、HbA1c、TC、TG、HDL-C、LDL-C水平均优于常规组,差异有统计学意义(P<0.05)。治疗后,两组患者的24 h尿蛋白定量与24 h尿微量白蛋白排泄率均低于本组治疗前,且糖肾方组患者的24 h尿蛋白定量(104.4±27.5)mg/d与24 h尿微量白蛋白排泄率(74.2±18.1)μg/min均低于常规组的(125.9±25.2)mg/d、(89.2±21.3)μg/min,差异有统计学意义(P<0.05)。结论自拟糖肾方治疗糖尿病肾病有较好的临床疗效。

Objective To study the efficacy of self-formulated Tangshen recipe in the treatment of diabetic nephropathy(DN).Methods A total of 84 patients with diabetic nephropathy were divided into Tangshen recipe group and conventional group according to random numerical table,with 42 cases in each group.The conventional group was treated with basic therapy,and the Tangshen recipe group was treated with self-formulated Tangshen recipe based on the basic therapy.The glucose and lipid metabolism indexes[fasting blood glucose(FBG),2 h postprandial glucose(2 h PG),glycosylated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)]and urinary albumin indexes(24 h urinary albumin excretion rate,24 h urinary protein quantitation)before and after treatment were compared between the two groups.Results After treatment,the levels of FBG,2 h PG,HbA1c,TC,TG,and LDL-C of the two groups were lower than those before treatment,and HDL-C was higher than that before treatment;the levels of FBG,2 h PG,HbA1c,TC,TG,HDL-C and LDL-C of Tangshen recipe group were better than those of conventional group.All the difference was statistically significant(P<0.05).After treatment,the 24 h urinary albumin excretion rate and 24 h urinary protein quantitation of the two groups were lower than those before treatment;the 24 h urinary protein quantitation(104.4±27.5)mg/d and 24 h urinary albumin excretion rate(74.2±18.1)μg/min of Tangshen recipe group were lower than(125.9±25.2)mg/d and(89.2±21.3)μg/min of conventional group.All the difference was statistically significant(P<0.05).Conclusion Self-formulated Tangshen recipe shows good clinical efficacy in the treatment of diabetic nephropathy.
