半导体激光辅助牙周基础治疗对中、重度慢性牙周炎的临床疗效观察 被引量:23

Observation of the curative effect of diode laser assisted periodontal initial therapy on moderate to severe periodontitis

摘要 目的:观察半导体激光辅助牙周基础治疗对中重度慢性牙周炎的治疗效果。方法:选取2018年3月~2019年12月大连市口腔医院特诊科收治的中、重度慢性牙周炎患者31例,随机分为激光组和器械组,每组患牙共计94颗,564个位点。器械组采用牙周基础治疗,激光组采用半导体激光辅助牙周基础治疗,分别于治疗前及治疗后2、3、6个月,比较两组患者牙周指标及螺旋体百分比(Spirochete%,S%);采用牙科畏惧量表(dental fear survey,DFS)、视觉模拟评分法(visnal analogue scale,VAS)评估患者治疗舒适度。结果:治疗后2、3个月激光组探诊出血(bleeding on probing,BOP)、探诊深度(probing depth,PD)及附着丧失(attachment loss,AL)、菌斑指数(plaque index,PLI)、出血指数(bleeding index,BI)、S%以及DFS、VAS评分均小于器械组(P<0.05);治疗后6个月激光组BOP、BI、DFS评分小于器械组(P<0.05)。结论:半导体激光辅助牙周基础治疗对中、重度慢性牙周炎治疗的短期效果更佳,能有效控制菌斑,减少牙龈出血,改善附着丧失,降低患者疼痛不适,提高牙周基础治疗疗效。 Objective:To observe the curative effect of diode laser assisted periodontal initial therapy on moderate to severe periodontitis.Methods:Thirty-one patients with moderate to severe periodontitis admitted to Dalian Stomatological Hospital from March 2018 to December 2019 were enrolled.The teeth were randomly divided into laser group and device group with a total of 94 teeth and 564 sites in each group.The device group received periodontal initial therapy,while the laser group received diode laser assisted periodontal initial therapy.The bleeding on probing(BOP),probing depth(PD),attachment loss(AL),plaque index(PLI),bleeding index(BI)and Spirochete%(S%)were compared between the two groups before and 2,3 and 6 months after treatment.The treatment comfort of patients was evaluated with dental fear survey(DFS)and visual analogue scale(VAS).Results:The laser group had lower PD,AL,PLI,BI,S%,DFS and VAS scores than the device group at 2 and 3 months after treatment(P<0.05).The laser group had lower AL,PD,BI and DFS scores than the device group at 6 months after treatment(P<0.05).Conclusion:The short-term curative effect of diode laser assisted periodontal initial therapy is better in the treatment of moderate to severe periodontitis.It can effectively control dental plaque,reduce gingival bleeding and improve attachment loss,alleviate discomfort and fear,which is beneficial to enhance the curative effect of periodontal initial therapy.
