膝骨关节炎中医证候与影像学的关系

To Study the Relationship Between TCM Syndromes of Knee Osteoarthritis and Imaging

目的分析膝骨关节炎中医证候与影像学的关系。方法选择2017年12月—2020年12月,收集的60例膝骨关节炎患者。依次对膝骨关节炎患者进行一系列的影像学诊断,分析不同膝骨关节炎的中医证候患者影像学的不同之处以及其不同证型与各种影像学表现之间存在的相关联系。结果 X线诊断方式侧重点为不同证型膝骨关节炎患者的膑骨位置、胫骨角、股胫角、股骨角、骨端囊性变以及关节间隙情况,可得到X线诊断对于膝骨关节炎中医证候的区分没有较为明显的临床价值(P> 0.05)。MRI诊断方式的侧重点是分析不同证型膝骨关节炎患者的关节游离体、骨质增生、积液、滑膜增厚、半月极损伤以及软骨损伤等的具体情况,可得到不同中医证型膝骨关节炎患者之间存在较大不同(P <0.05)。结论两种疾病诊断方式均适用于膝骨关节炎患者的临床推广,但是MRI诊断结果与其中医证型存在相关联系,可为后续疾病干预治疗方案的制定提供临床依据。

Objective To study and analyze the relationship between TCM syndromes and imaging of knee osteoarthritis.Methods From December 2017 to December 2020,60 patients with knee osteoarthritis were collected.A series of imaging diagnoses were carried out on patients with knee osteoarthritis in turn,and the differences in imaging of patients with TCM syndromes of different knee osteoarthritis and the correlation between different syndrome types and various imaging manifestations were analyzed.Results The X-ray diagnosis method focuses on the position of the patella,tibial angle,femoral tibial angle,femoral angle,end cystic degeneration,and joint space of patients with different syndromes of knee osteoarthritis.X-ray diagnosis can be obtained for knee bone There is no obvious clinical value in distinguishing TCM syndromes of arthritis(P >0.05).The focus of the MRI diagnosis method is to analyze the specific conditions of joint loose body,bone hyperplasia,effusion,synovial thickening,semilunar injury and cartilage injury in patients with different syndromes of knee osteoarthritis.Different TCM syndromes can be obtained.There are big differences among patients with knee osteoarthritis(P < 0.05).Conclusion Both disease diagnosis methods are suitable for the clinical promotion of knee osteoarthritis patients,but there is a correlation between the MRI diagnosis results and the Chinese medical syndromes,which can provide clinical basis for subsequent disease intervention and treatment plans.
