从肝脾肾治疗帕金森病立论基础

Discussion on Treatment of Parkinson’s Disease from Liver,Spleen and Kidney

帕金森病,是一种主要由黑质多巴胺缺乏引起的以静止性震颤、运动迟缓、肌强直等为主要临床表现的运动障碍性疾病,近年来患病人数呈缓慢上升趋势,尤其以50岁以上并伴有中风、脑梗死等基础病的中老年群体为主。病因病机复杂多样,可能与感染、遗传性、中毒等因素有关,目前为止,西医发病机制尚不明确,可能为遗传因素、环境因素与神经系统老化等多种因素共同作用的结果。根据中老年人群脏腑渐衰和气血津液渐亏、虚久成瘀、瘀血阻络的特点,该病主要以肝肾亏虚为病机之基,以瘀血阻络为病机之要,以脾胃虚弱为病机关键,因此,帕金森病的治疗当以培补肝肾、调理脾胃、活血化瘀、疏经通络、调畅情志等为着眼点,以防病情进一步发展,改善患者预后,提高患者生活质量。近年来随着对帕金森病的不断深入研究,中医药在改善帕金森病患者的临床症状和预后方面发挥着日趋重要的作用,中医在分析病因病机的基础上,以整体观念和辨证论治为基本原则,通过研习和阅读相关文献,结合临床实践,为帕金森病的现代中医专病提供治疗思路,使得该病在中医药治疗方面取得创新与突破。近年来通过从肝、脾、肾、瘀论治,在治疗帕金森病临床症状方面取得了较为良好的效果,以下对中医药治疗帕金森病的文献报道及临床用药疗效进行归纳综述。

Parkinson’s disease is a kind of dyskinesia disease,which is mainly caused by the lack of dopamine in substantia nigra.In recent years,the number of patients is increasing slowly,especially the middle-aged and old people who are over50 years old with basic diseases such as stroke and cerebral infarction.The etiology and pathogenesis are complex and diverse,which may be related to infection,heredity,poisoning and other factors.So far,the pathogenesis of Western medicine is not clear,which may be the result of the interaction of genetic factors,environmental factors and nervous system aging and other factors.According to the characteristics of the aging people’s viscera,blood and body fluid,blood stasis and blood stasis blocking collaterals,the disease is mainly based on the deficiency of liver and kidney,blood stasis blocking collaterals as the main pathogenesis,and the deficiency of spleen and stomach as the key pathogenesis.Therefore,the treatment of Parkinson’s disease should focus on the cultivation of liver and kidney,the regulation of spleen and stomach,the activation of blood circulation and resolving blood stasis,the dredging of meridians and collaterals and the regulation of emotions,so as to prevent and treat the disease as well as improve the prognosis and the quality of life of patients.In recent years,with the continuous in-depth study of Parkinson’s disease,traditional Chinese medicine plays an increasingly important role in improving the clinical symptoms and prognosis.Based on the analysis of the etiology and pathogenesis,traditional Chinese medicine takes the overall concept and syndrome differentiation and treatment as the basic principle,through studying and reading the relevant literature,combined with clinical practice,it provides treatment ideas for modern traditional Chinese medicine of Parkinson’s diseaseso that the treatment of traditional Chinese medicine has achieved innovation and breakthrough.In recent years,through the treatment of liver,spleen,kidney and blood stasis,we have achieved good results in the treatment of clinical symptoms of Parkinson’s disease.
