Impact of Centralized Volumetric Procurement Policy on Drug Supply Assurance

目的:分析集中带量采购政策对药品供应保障存在的隐患,为今后的集采提出相关政策的完善措施。方法:依据药品供应链理论,基于采购政策制度设计和实行效果,探究涉及药品短缺风险的环节。结果:中标价大幅下降,原料药稳定供应存在隐患;独家配送不能保障基层地区药品可获得性;较大的地区差价易产生供应隐患,行政倾向的用药方式无法满足患者多样化需求。结论:针对以上影响药品供应保障的风险因素,提出完善相关原料药政策,有序保障中选品种的生产与供给;提高配送商遴选标准,合理规划配送方式提高基层地区配送能力;缩小特殊品种地区间差价,实行梯度降价等方式满足临床用药需求的建议。

Objective: To analyze the hidden danger in drug supply assurance brought by the centralized volumetric procurement policy, and propose relevant measures for future procurement. Methods: According to the theory of drug supply chain, the links involved in the risk of drug shortage were explored based on the design and implementation effects of the procurement policy system. Results: The winning bid price had dropped significantly, and there were hidden dangers in the stable supply of APIs, exclusive distribution could not ensure the availability of drugs in grassroots areas, large regional price differences were easy to induce hidden dangers in supply, and administratively inclined medication methods could not meet the diversified needs of patients. Conclusion: In view of the above risk factors affecting drug supply guarantee, it is recommended to improve relevant API policies and orderly ensure the production and supply of the selected varieties, improve the selection criteria for distributors and reasonably plan the distribution mode to improve the distribution capacity in grassroots areas;narrow the price difference between regions of special varieties and implement gradient price reduction to meet the needs of clinical medication.
