摘要
目的探讨超声评估晚孕期妊高征母胎心功能及胎儿生长发育的价值。方法选取2019年9月至2020年9月60例孕龄≥28周的孕妇,其中妊高征的产妇30例列入观察组,孕检正常的孕妇30例列入对照组,通过多普勒超声比较两组孕妇的心功能情况,胎儿左、右室Tei指数及生长发育。结果两组孕妇左心射血分数(LVEF)比较,差异无统计学意义(P>0.05),观察组左心室舒张末期内径(LVDd)、左心房内径(LAD)高于对照组,左室短轴缩短率(LVFS)低于对照组,差异有统计学意义(P<0.05)。观察组胎儿左心室的心室等容收缩时间(ICT)+心室等容舒张时间(IRT)、Tei指数和右心室ICT+IRT、Tei指数均高于对照组胎儿,左心室的射血时间(ET)、右心室ET低于对照组胎儿,差异有统计学意义(P<0.05)。观察组胎儿的头围、双顶径、腹围均小于对照组的胎儿,差异有统计学意义(P<0.05)。结论通过多普勒超声获得晚孕期妊高征母胎心功能及胎儿生长发育指标,及时监测母婴情况,为避免不良妊娠结局提供科学监测的参考依据。
Objective To investigate the value of maternal fetal cardiac function and fetal growth and development in late pregnancy induced hypertension by ultrasonic evaluation.Methods A total of 60 puerperae with gestational age≥28 weeks from September 2019 to September 2020 were selected,and they were divided into the observation group(with pregnancy induced hypertension,n=30)and the control group(with normal pregnancy test,n=30).The cardiac function of pregnant women,Tei index and growth and development of the fetal left and right ventricles of fetus were compared between the two groups of puerperae by Doppler ultrasonography.Results There was no statistically significant difference in left ventricular ejection fraction(LVEF)between the pregnant women of two groups(P>0.05).The left ventricular end diastolic diameter(LVDd)and left atrial diameter(LAD)in the observation group were higher than those in the control group,and the left ventricular fraction shortening(LVFS)in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).The left and right ventricular isovolumic contraction time(ICT),ventricular isovolumic relaxation time(IRT),Tei index in the fetus of observation group were all higher than those in the control group,while the left ventricular ejection time(ET)and right ventricular ET were lower than those in the control group,with statistically significant differences(P<0.05).The head circumference,biparietal diameter and abdominal circumference of the fetus in the observation group were all smaller than those in the control group,with statistically significant differences(P<0.05).Conclusion The maternal fetal cardiac function and fetal growth and development in late pregnancy induced hypertension were obtained by Doppler ultrasonography.And the situation of mother and infant was monitored in time to provide scientific monitoring references for avoiding adverse pregnancy outcomes.
