穴位按摩联合五行音乐疗法对胃癌根治术后患者睡眠质量和生活质量的影响

Impacts of acupoint massage combined with five elements music therapy on sleep quality and quality of life in patients after radical gastrectomy for gastric cancer

目的分析穴位按摩联合五行音乐疗法对胃癌根治术后患者睡眠质量和生活质量的影响,探讨一种有效的护理模式,以提高护理质量。方法选取2019年9月至2020年9月深圳市中西医结合医院普外科120例胃癌根治术后患者,随机分为对照组和干预组,每组各60例,对照组予以围术期常规护理,干预组在对照组基础上实施穴位按摩及五行音乐疗法联合护理,比较两组的负性情绪[汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)]、睡眠质量[匹兹堡睡眠质量指数(PSQI)]、生活质量[癌症患者生活质量测定量表(QLQ-LC43评分)]。结果干预组出院时、出院后1个月的HAMA评分与HAMD评分均低于对照组,差异有统计学意义(P<0.05);干预组出院时、出院后1个月的PSQT评分均低于对照组,差异有统计学意义(P<0.05);干预组出院时、出院后1个月的QLQ-LC43评分均高于对照组,差异有统计学意义(P<0.05)。结论穴位按摩联合五行音乐疗法在胃癌根治术后护理中的应用效果理想,可明显减轻患者的负性情绪,改善其睡眠质量与生活质量,值得推广与应用。

Objective To analyze the impacts of acupoint massage combined with five elements music therapy on sleep quality and quality of life(QoL)in patients after radical gastrectomy for gastric cancer,so as to investigate an effective nursing mode and improve the nursing quality.Methods A total of 120 patients in the Department of General Surgery admitted to Shenzhen Hospital of Integrated Traditional Chinese and Western Medicine from September 2019 to September 2020 were selected and randomly divided into the control group(n=60)and the intervention group(n=60).The control group was treated with conventional nursing during perioperative period,while the intervention group was treated with acupoint massage and five-element music therapy combined nursing on the basis of the control group.The negative emotions[Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD)],sleep quality[Pittsburgh sleep quality index(PSQI)]and quality of life[quality of life scale for cancer patients(QLQ-LC43 score)]of the two groups were compared.Results The HAMA and HAMD scores of the intervention group at discharge and 1 month after discharge were all lower than those of the control group,with statistically significant differences(P<0.05).The PSQI scores of the intervention group at discharge and 1 month after discharge were all lower than those of the control group,with statistically significant differences(P<0.05).The QLQ-LC43 scores of the intervention group at discharge and 1 month after discharge were higher than those of the control group,with statistically significant differences(P<0.05).Conclusion The acupoint massage combined with five elements music therapy has a satisfying efficacy in nursing after radical gastrectomy for gastric cancer,which can obviously reduce patients'negative emotions and improve their sleep quality and QoL.Therefore,it is worthy of promotion and application.
