四川省分级诊疗实施现状分析

Implementation status of hierarchical diagnosis and treatment in Sichuan Province

目的分析四川省分级诊疗实施现状,探索分级诊疗实施困境,为各省构建合理有序的就医秩序提供参考。方法对医院和基层医疗卫生机构卫生资源配置和卫生服务水平进行对比分析,并利用Pearson相关系数对两者进行相关性分析。结果医院各类卫生资源增长速度明显高于基层医疗卫生机构;2018年基层医疗卫生机构诊疗、住院服务量占比仅为57.68%和26.59%;卫生资源配置与卫生服务水平呈正相关。结论四川省分级诊疗实施效果与预期存在较大差距,而医院的快速发展抑制了基层卫生服务水平的提高。

Objective To analyze the implementation status of hierarchical diagnosis and treatment in Sichuan Province,to explore the implementation dilemma of hierarchical diagnosis and treatment,and to provide reference for other provinces to construct an orderly medical treatment environment.Methods A comparative analysis was made on the allocation of health resources and health service levels between hospitals and primary healthcare institutions,and the correlation was analyzed using Pearson’s correlation coefficient.Results The growth rate of various health resources in hospitals was significantly higher than that of primary healthcare institutions.In 2018,the proportions of diagnosis and hospitalization services in primary healthcare institutions were 57.68%and 26.59%respectively.The allocation of health resources was positively correlated with the level of health services.Conclusions There is a big gap between the implementation effect of hierarchical diagnosis and treatment and the expectations in Sichuan Province,and the rapid development of hospitals inhibits the improvement of health servicesin primary healthcare institutions.
