维生素D补充治疗对肥胖型多囊卵巢综合征患者糖脂代谢的影响 被引量:6

Effect of Vitamin D Supplementation Treatment on Glucose and Lipid Metabolism in Polycystic Ovary Syndrome Patients with Obese

摘要 目的:探讨维生素D补充治疗对肥胖型多囊卵巢综合征(PCOS)患者糖脂代谢的影响。方法:选取我院2017年5月—2019年12月接受治疗并完成随访的76例肥胖型PCOS患者为研究对象,按照随机数字表均分为对照组和维生素D补充治疗组,两组患者年龄、BMI间均无差异(P>0.05),具有可比性。对照组患者采用常规二甲双胍和达英-35治疗,同时给予适当干预进行体重控制。维生素D干预组在对照组基础上,给予维生素D(400 IU/粒)口服,2粒/次,1次/日。干预周期为12周。记录患者治疗前后身高体重,计算BMI,比较治疗前后两组BMI的变化。治疗前后抽取两组患者空腹静脉血,检测血清维生素D、TC、TG、HDL-C、LDL-C、FPG、FINS水平,计算HOMA-IR,比较治疗前后两组患者血清维生素D水平和糖脂代谢变化,以及两组患者治疗后血清维生素D水平和糖脂代谢的变化。结果:经过12周治疗后,两组患者BMI均显著下降(P<0.05),且干预前后两组患者BMI均无差异(P>0.05)。治疗后,维生素D补充治疗组患者血清维生素D水平显著高于对照组患者,且两组患者血清TC、TG、LDL-C、FPG、FINS、HOMA-IR水平较治疗前显著降低(P<0.05),血清HDL-C水平显著增高(P<0.05),且维生素D补充治疗组患者血清TC、TG、LDL-C、FPG、FINS、HOMA-IR均显著低于对照组患者,HDL-C显著高于对照组患者,差异均有统计学意义(P<0.05)。结论:维生素D补充治疗可改善患者糖脂代谢,有利于PCOS患者病情控制,值得在临床推广应用。 Objective To explore the effect of vitamin D supplementation on glucose and lipid metabolism in polycystic ovary syndrome(PCOS)patients with obese.Method Totally 76 obese PCOS patients who received treatment and completed follow-up in our hospital from May 2017 to December 2019 were selected as the research objects.According to the random number table,they were divided into a control group and a vitamin D supplement treatment group.There was no difference in age and BMI between the two groups of patients(P>0.05),which was comparable.Patients in the control group were treated with metformin and Diane-35,and given appropriate interventions for weight control.On the basis of the control group,the vitamin D intervention group was given vitamin D(400 IU/capsule)orally,2 capsules/time,once/day.The intervention period was 12 weeks.Record the patient’s height and weight before and after treatment,calculate BMI,and compare the changes between the two groups before and after treatment.Fasting venous blood was drawn from two groups of patients before and after treatment to detect serum vitamin D,TC,TG,HDL-C,LDL-C,FPG and FINS levels,calculate HOMA-IR,and compare the changes in serum vitamin D levels and glucose and lipid metabolism of the two groups of patients before and after treatment,and the changes in serum vitamin D levels and glucose and lipid metabolism of the two groups after treatment.Result After 12 weeks of treatment,the BMI of the two groups decreased significantly(P<0.05),and there was no difference in BMI between the two groups before and after the intervention(P>0.05).After treatment,serum vitamin D levels in the vitamin D supplementation treatment group were significantly higher than those in the control group.And after treatment,serum TC,TG,LDL-C,FPG,FINS and HOMA-IR levels of the two groups were significantly lower than before treatment(P<0.05),serum HDL-C levels were significantly increased(P<0.05),and in the vitamin D supplementary treatment group,serum TC,TG,LDL-C,FPG,FINS and HOMA-IR were significantly lower than those in the control group,HDL-C was significantly higher than that of the control group patients,the difference was statistically significant(P<0.05).ConclusionVitamin D supplementation treatment can ameliorate the glucose and lipid metabolism of patients,and which is beneficial to control patient’s condition.It is worthy of clinical application.
