腔镜辅助甲状腺肿瘤微创手术的临床疗效观察

Efficacy of endoscopic-assisted minimally invasive surgery for thyroid tumors

目的探讨腔镜辅助甲状腺肿瘤微创手术的临床应用效果。方法选取2019年1月至2020年1月间上海市浦东新区浦南医院收治的60例甲状腺肿瘤患者,根据手术方式不同分为观察组和对照组,每组30例。观察组患者采用腔镜辅助甲状腺肿瘤微创手术治疗,对照组患者采用常规开放性手术治疗,比较两组患者手术时间、住院时间、颈部皮肤紧张评分、颈部皮肤感觉减退评分及术后并发症发生情况。结果观察组患者手术时间和住院时间短于对照组,术中出血量和术后引流量低于对照组,切口长度短于对照组,差异均有统计学意义(均P<0.05)。观察组患者颈部皮肤紧张评分和颈部皮肤感觉减退评分均低于对照组,差异均有统计学意义(均P<0.05)。完成手术后,两组患者均未出现喉返神经损伤,观察组患者并发症发生率为6.7%,低于对照组患者的16.7%,差异有统计学意义(P<0.05)。结论甲状腺肿瘤采用腔镜辅助开展微创手术,具有较高的安全性,治疗效果好,值得推广。

Objective To analyze the clinical efficacy of endoscopic-assisted minimally invasive surgery for thyroid tumors.Methods Sixty patients with thyroid tumors and who received treatment at Punan Hospital of Pudong New Area were selected from January 2019 to January 2020.They classified into an observation group and a control group with 30 patients in each group according to the operation methods.The observation group underwent endoscopic-assisted minimally invasive surgery and the control group underwent conventional abdominal surgery.Duration of operation,length of hospital stay,neck skin tension score and neck skin hypoesthesia score and postoperative complications were compared between the two groups.Results The duration of operation and length of hospital stay,intraoperative blood loss,postoperative drainage volume,incision length neck skin tension score and neck skin hypoesthesia score were lower in the observation group than those in the control group(all P<0.05).No patient had recurrent laryngeal nerve injury after the surgery.The incidence of postoperative complications was 6.7%in the observation group which significantly lower than 16.7%of the control group(P<0.05).Conclusion Endoscopic-assisted minimally invasive surgery shows high safety and efficacy in the treatment of thyroid tumors.It is worthy of promotion.
