摘要
目的探讨经阴道三维超声自由解剖成像(OmniView)技术对卵巢癌的诊断价值。方法选取2017年1月至2020年6月间宝鸡高新医院收治的60例卵巢癌患者作为卵巢癌组,同期选取60例卵巢良性肿瘤患者作为良性组。所有患者都给予常规超声与经阴道三维超声OmniView成像检查,记录影像学特征并判断诊断价值。方法卵巢癌组的高回声、囊内隔增厚、后壁内衬、乳头状实质比例与病灶最大径高于良性组,差异均有统计学意义(均P<0.05)。卵巢癌组的收缩期最高峰值血流流速及舒张期最低峰值血流流速均高于良性组,阻力指数低于良性组,差异均有统计学意义(均P<0.05)。两组患者的搏动指数比较,差异无统计学意义(P>0.05)。经阴道三维超声OmniView成像诊断卵巢癌的灵敏度与特异度分别为91.7%和98.3%,临床分期诊断准确性为95.0%。结论经阴道三维超声OmniView成像技术可在常规超声的基础提供更多的卵巢相关诊断信息,对卵巢癌及其临床分期的诊断准确性比较高。
Objective To explore the diagnostic value of transvaginal three-dimensional ultrasound imaging of OmniView for ovarian cancer.Methods From January 2017 to June 2020,60 patients with ovarian cancer diagnosed and treated at Baoji High-Tech Hospital were selected as the ovarian cancer group,and 60 patients with benign ovarian tumors were selected as the benign group during the same period.All patients received conventional ultrasound and transvaginal three-dimensional ultrasound imaging of Omni-View examination.Imaging characteristics were recorded and diagnostic value was assessed.Results The proportion of hyperechoic,thickened intracapsular septum,posterior wall lining,papillary parenchyma and the largest diameter of lesions were higher in the ovarian cancer group than in the benign group(all P<0.05).The peak systolic velocity and minimum diastolic velocity were were higher in the ovarian cancer group than in theb benign group and the resistance index were lower in the ovarian cancer group than in theb benign group(all P<0.05).There was no significant difference in the pulsation index between the two groups(P>0.05).The sensitivity and specificity of diagnosis were 91.7%and 98.3%,respectively,and the accuracy of clinical staging were 95.0%for transvaginal three-dimensional ultrasound imaging of Omni-View.Conclusion Transvaginal three-dimensional ultrasound imaging of OmniView can provide additional ovarian-related diagnostic information compared with conventional ultrasound with relatively high diagnostic accuracy for ovarian cancer and clinical staging.
