完全腹膜外腹腔镜精索静脉高位结扎术治疗精索静脉曲张的效果探讨

Application value of total extraperitoneal laparoscopic high ligation of the spermatic vein in the treatment of varicocele

目的探讨完全腹膜外腹腔镜精索静脉高位结扎术治疗精索静脉曲张的临床效果。方法80例精索静脉曲张患者,随机分为对照组和观察组,各40例。对照组采取腹腔镜经腹腔精索静脉高位结扎术治疗,观察组采取完全腹膜外腹腔镜精索静脉高位结扎术治疗。比较两组患者治疗后各手术相关指标及并发症发生率。结果观察组手术时间(30.23±2.37)min、住院时间(5.96±1.34)d及最早下床活动时间(1.06±0.01)d均短于对照组的(42.39±2.65)min、(8.36±1.62)d、(3.16±0.42)d,术中出血量(5.96±1.34)d少于对照组的(8.36±1.62)d,差异具有统计学意义(P<0.05)。观察组并发症发生率为5.00%,显著低于对照组的32.50%,差异具有统计学意义(P<0.05)。结论完全腹膜外腹腔镜精索静脉高位结扎术治疗精索静脉曲张的效果较好,可有效改善患者各临床指标,促进恢复,减少治疗后并发症发生率,值得临床推广应用。

Objective To discuss the clinical effect of total extraperitoneal laparoscopic high ligation of the spermatic vein in the treatment of varicocele.Methods A total of 80 patients with varicocele were randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with laparoscopic high ligation of the peritoneal spermatic vein,and the observation group was treated with total extraperitoneal laparoscopic high ligation of the spermatic vein.The surgical related indicators and occurrence of complications after treatment were compared between the two groups.Results The operation time(30.23±2.37)min,hospitalization time(5.96±1.34)d and first off-bed activity time(1.06±0.01)d of the observation group were shorter than(42.39±2.65)min,(8.36±1.62)d and(3.16±0.42)d of the control group,and intraoperative blood loss(5.96±1.34)d was less than(8.36±1.62)d of the control group.All the difference was statistically significant(P<0.05).The incidence of complications of the observation group was 5.00%,which was significantly lower than 32.50%of the control group,and the difference was statistically significant(P<0.05).Conclusion Total extraperitoneal laparoscopic high ligation of the spermatic vein has good effect in the treatment of varicocele,which can effectively improve the clinical indicators of patients,promote their recovery,and lower the incidence of complications.It is worthy of clinical promotion and application.
