中医师承教育在中医住院医师规范化培训中的作用

The Role of Master-Apprentice Education of Traditional Chinese Medicine in the Standardized Training for Residents of Traditional Chinese Medicine

目的分析中医师承教育在中医住院医师规范化培训中的作用。方法选取2018年4月—2020年4月福建省泉州市中医院498名中医住院医师为研究对象,按照培训方式的不同,将2018年4月—2019年3月进行传统规范化培训的248名中医住院医师作为对照组,将2019年4月—2020年3月进行中医师承教育的250名中医住院医师作为观察组。培训结束后,应用自制中医师承教育式规范化培训满意度问卷、迷你临床演练评估(Mini-clinical evaluation exercise,Mini-CEX)和临床操作直接观察评估(Directly observed procedural skills,DOPS)评价2组中医住院医师培训满意度、临床基础能力和临床操作技能。结果观察组在学习积极性、自主学习能力、临床思维能力和分析与解决问题能力等方面满意度明显高于对照组(P<0.05);观察组Mini-CEX和DOPS的得分、及格率、优秀率明显高于对照组(P<0.05)。结论中医师承教育在中医住院医师规范化培训中,可显著提高住院医师的学习能力和专业水平,具有很好的应用价值。

Objective To analyze the role of master-apprentice education of traditional Chinese medicine(TCM)in the standardized training for residents of TCM.Methods 498 residents of TCM of our hospital from April 2018 to April 2020 were selected as the subjects of this study.According to different training methods,248 residents of TCM who received traditional standardized training from April 2018 to March 2019 were taken as the control group,and 250 residents of TCM who received master-apprentice education of TCM from April 2019 to March 2020 were taken as the observation group.At the end of the training,the two groups of residents of TCM were investigated on their satisfaction with the training by using the self-made questionnaire.The mini-Clinical Evaluation Exercise(MINI-CEX)and the Direct Observation of Procedural Skills(DOPS)were used to assess the basic clinical competence and clinical operation skills of residents.Results The satisfaction of the observation group was significantly higher than that of the control group in learning enthusiasm,independent learning ability,clinical thinking ability and ability to analyze and solve problems(P<0.05).The score,pass rate and excellence rate of MINI-CEX and DOPS in the observation group were significantly higher than that in the control group(P<0.05).Conclusion In the standardized training for residents of TCM,the master-apprentice education of TCM can significantly improve the learning ability and professional level of residents,which has a good application value.
