早期NSCLC患者全胸腔镜肺叶切除术的临床应用研究进展

Advances in clinical application of total thoracoscopic lobectomy in early NSCLC patients

肺癌是世界上患病率和死亡率增长最快的恶性肿瘤,对人类的健康和生命有巨大威胁,其中非小细胞肺癌(NSCLC)的占比约为80%~85%,需引起高度重视。对早期NSCLC患者应积极实施根治术治疗以延缓病情发展,延长生存期。随着微创技术的发展和医疗设备的更新,全胸腔镜肺叶切除术作为一种微创术式已经在早期NSCLC患者临床治疗中得到了广泛的应用,且其相较于传统开放手术的优势也已得到该领域工作者的一致认可。本文首先探讨全胸腔镜肺叶切除术的解剖学基础,总结其适应证和禁忌证和手术流程,然后重点探讨其在早期NSCLC患者中的优劣,以期为临床治疗方案的选择提供系统化的参考。未来,应拓宽该术式的适应证,改进和优化手术切口的选择、更新术前和术中定位方法,进一步探讨该术式对远期生存率的影响,还应关注如何降低手术难度和费用。

Lung cancer is one of the malignant tumors with the fastest growth in morbidity and mortality in the world.It poses a great threat to human health and life.Among them,non-small cell lung cancer(NSCLC)accounts for about 80%~85%,which should be paid great attention to.Early NSCLC patients should be actively treated with radical operation to delay the development of the disease and prolong the survival time.With the development of minimally invasive technology and the renewal of medical equipment,total thoracoscopic lobectomy,as a minimally invasive procedure,has been widely used in the clinical treatment of early NSCLC patients,and its advantages over traditional open surgery have also been unanimously recognized by workers in this field.This paper first explores the anatomical basis of total thoracoscopic lobectomy,summarizes its indications,contrain dications and surgical procedures,and then focuses on its advantages and disadvantages in early NSCLC patients,with a view to providing a systematic reference for clinical treatment options.In the future,we should broaden the indications of the operation,improve and optimize the choice of incision,update the preoperative and intraoperative positioning methods,further explore the effect of the operation on the long-term survival rate,and pay attention to how to reduce the difficulty and cost of the operation.
