胰岛素联合达格列净治疗2型糖尿病患者的临床价值

The clinical value of insulin combined with dapagliflozin in the treatment of type 2 diabetes

目的探究胰岛素联合达格列净治疗2型糖尿病患者的临床价值。方法选取2019年1月至2021年1月本院收治的100例2型糖尿病患者作为研究对象,采用随机数字表法分为观察组和对照组,每组50例。对照组采用胰岛素治疗,观察组在对照组的基础上采用达格列净治疗。比较两组临床疗效、不良反应发生率、治疗前后空腹血糖(FPG)水平、餐后2 h血糖(2 h PG)水平、血糖达标时间、胰岛素用量和尿蛋白定量情况及糖化血红蛋白(HbA1c)、体重指数(BMI)、甘油三酯(TG)、胆固醇(TC)水平。结果两组不良反应发生率比较差异无统计学意义;治疗后,观察组治疗总有效率明显高于对照组(P<0.05);观察组FPG、2 h PG水平均明显低于对照组(P<0.05);观察组血糖达标时间短于对照组,胰岛素用量、尿蛋白定量均明显低于对照组(P<0.05);观察组HbA_(1)c、BMI、TG、TC均明显低于对照组(P<0.05)。结论胰岛素联合达格列净治疗2型糖尿病效果显著,可有效降低患者血糖水平及胰岛素用量,保证用药安全性和有效性,提高患者的生活质量和满意度,提升医院整体医疗服务水平,值得推广应用。

Objective To explore the clinical value of insulin combined with dapagliflozin in the treatment of type 2 diabetes.Methods 100 patients with type 2 diabetes who were admitted to our hospital from January 2019 to January 2021 were selected as the research subjects.They were divided into observation group and control group by random number table method,with 50 cases in each group.The control group was treated with insulin,and the observation group was treated with dagliazin on the basis of the control group.The clinical curative effect,the incidence of adverse reactions,fasting blood glucose(FPG)levels before and after the treatment,2 h postprandial blood glucose(2 h PG)levels,the time of blood glucose reaching the target,the insulin dosage,the urinary protein quantitative situation,and the sugar hemoglobin(HbA_(1)c),body mass index(BMI),triglycerides(TG),cholesterol(TC)level were compared between two groups.Results There was no significant difference in the incidence of adverse reactions between the two groups.After treatment,the total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The levels of FPG and 2 h PG in observation group were significantly lower than those in control group(P<0.05).The time of blood glucose reaching the target in the observation group was shorter than that in the control group,and the insulin dosage and urinary protein in the observation group were significantly lower than those in the control group(P<0.05).HbA1c,BMI,TG and TC in observation group were significantly lower than those in control group(P<0.05).Conclusion Insulin combined with dapagliflozin has significant effect on the treatment of type 2 diabetes mellitus,which can effectively reduce the blood glucose level and insulin dosage,ensure the safety and effectiveness of medication,improve the quality of life and satisfaction of patients,and improve the overall medical service level of the hospital.It is worthy of clinical popularization and application.
