尿NGAL水平检测在原发性高血压患者肾功能损害诊断中的应用 被引量:1

摘要 目的探讨尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平检测在原发性高血压患者肾功能损害诊断中的临床应用价值。方法选取2018年9月至2019年3月本院收治的50例原发性高血压患者作为观察组,并根据尿白蛋白排泄率(UAER)分为正常白蛋白尿组(NA组,UAER<30 mg/24 h,n=18)、微量白蛋白尿组(MA组,30 mg/24 h≤UAER≤300 mg/24 h,n=21)、临床白蛋白尿组(CN组,UAER>300 mg/24 h,n=11),另选取同期健康体检者30名作为对照组,对上述入选者分别应用美国贝克曼DXC-800全自动生化分析仪测定血清肌酐(SCr)水平,酶联免疫双抗体夹心法检测尿NGAL水平,经Cockroff-Gault公式计算肾小球滤过率(eGFR),分析检测结果及尿NGAL分别与SCr和eGFR的相关性。结果观察组尿NGAL和SCr水平均高于对照组,eGFR水平低于对照组,差异有统计学意义(P<0.05);经Pearson相关性分析发现,尿NGAL水平与SCr呈正相关(r>0,P<0.05),与eGFR无相关性(r<0)。结论尿NGAL水平检测在原发性高血压患者肾功能损害诊断中具有一定应用价值,可准确反映肾损害程度,对临床原发性高血压患者肾功能损害诊断和治疗具有重要指导意义。 Objective To investigate the clinical value of serum neutrophil gelatinase-associated lipid carrier protein(NGAL)in the diagnosis of renal impairment in patients with essential hypertension.Methods 50 patients with essential hypertension admitted to our hospital from September 2018 to March 2019 as the observation group,and according to urinary albumin excretion rate(UAER),they were divided into normal albuminuria group(NA group,UAER<30 mg/24 h,n=18)and microalbuminuria group(MA group,30 mg/24 h≤UAER≤300 mg/24 h,n=21),clinical albuminuria group(CN group,UAER>300 mg/24 h,n=11).Another 30 healthy subjects were selected as the control group during the same period.The American Beckman DXC-800 automatic biochemical analyzer was used to determine the serum creatinine(SCr)level and enzyme-linked immunoassay.The double-antibody sandwich method was used to detect the level of urine NGAL,the glomerular filtration rate(eGFR)was calculated by Cockroff-Gault formula,and the test results and the correlation between urine NGAL and SCr and eGFR.Results The urine NGAL and SCr levels of the observation group were higher than those of the control group,and the eGFR level was lower than that of the control group,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that the level of urine NGAL was positively correlated with SCr(r>0,P<0.05),and there was no correlation with eGFR(r<0).Conclusion Urine NGAL level detection has certain application value in the diagnosis of renal impairment in patients with essential hypertension,which can accurately reflect the degree of renal impairment,and has important guiding significance for the diagnosis and treatment of renal impairment in patients with clinical essential hypertension.
