摘要
目的:分析核磁胰胆管成像在肝外胆结石诊断中的应用效果。方法:选取2019年2月~2020年6月到本院进行诊疗的110例疑似肝外胆结石患者为研究对象,手术病理诊断结果显示阳性患者95例,阴性患者15例。对所有患者均开展腹部CT检查和核磁胰胆管成像检查,观察两种方法对不同大小肝外胆结石诊断准确率,观察两种方法的诊断敏感性、特异性和准确性。结果:对于直径>9mm的肝外胆结石,核磁胰胆管成像诊断准确率为100.00%(35/35),显著高于腹部CT的80.00%(28/35),P<0.05;对于直径≤9mm的肝外胆结石,核磁胰胆管成像诊断准确率为96.67%(58/60),显著高于腹部CT的73.33%(44/60),P<0.05。核磁胰胆管成像诊断敏感性为97.89%(93/95),诊断特异性为100.00%(15/15),诊断准确性为98.18%(108/110);腹部CT诊断敏感性为75.79%(72/95),诊断特异性为60.00%(9/15),诊断准确性为73.64%(81/110),组间差异显著(P<0.05)。结论:相比腹部CT,核磁胰胆管成像在肝外胆结石诊断中具有更好的诊断效果。
Objective:To analyze the application effect of nuclear MRCP in the diagnosis of extrahepatic gallstones.Methods:Select 110 patients with suspected extrahepatic gallstones who came to our hospital for diagnosis and treatment from February 2019 to June 2020 as the research objects.Pathological diagnosis showed that 95 cases were positive and 15 cases were negative.Abdominal CT examination and nuclear MRCP were performed for all patients.The diagnostic accuracy of the two methods for different sizes of extrahepatic gallstones was observed,and the diagnostic sensitivity,specificity and accuracy of the two methods were observed.Results:For extrahepatic gallstones with diameter>9mm,the diagnostic accuracy of MR cholangiopancreatography was 100.00%(35/35),which was significantly higher than the 80.00%(28/35)of abdominal CT,P<0.05;for diameter≤9mm For extrahepatic gallstones,the diagnostic accuracy of MR cholangiopancreatography was 96.67%(58/60),which was significantly higher than 73.33%(44/60)of abdominal CT,P<0.05.The diagnostic sensitivity,specificity and accuracy of MRCP were 97.89%(93/95),100.00%(15/15)and 98.18%(108/110),The sensitivity,specificity and accuracy of abdominal CT were 75.79%(72/95),60.00%(9/15)and 73.64%(81/110),respectively,with significant difference between the two groups(P<0.05).Conclusion:Compared with abdominal CT,MRCP has better diagnostic effect in the diagnosis of extrahepatic gallstones.
