彩色多普勒联合超声血管造影对自体动静脉内瘘狭窄血管PTA治疗手术前后评估及临床效果比较

Evaluation and Clinical Effect Comparison of Color Doppler Ultrasonography Combined with Ultrasound Angiography in the Treatment of Autogenous Arteriovenous Impotence After PTA Operation

目的:探讨彩色多普勒联合超声血管造影对自体动静脉内瘘狭窄血管PTA治疗手术前后评估及临床效果比较。方法:选择2019年9月~2020年9月在本院长期维持血液透析且因自体动静脉内瘘狭窄行血管PTA手术的86例患者作为研究对象。对患者使用彩色多普勒联合超声血管造影对患者透析用血管进行长期评估,并进行相应的诊断和处理,与数字减影技术评估透析用血管结果进行对比,并对相关数据进行统计学分析。结果:各型患者在手术之后,血管内径数值都有一定上升,血流量也有明显改善,术前术后差异有统计学意义(P<0.05);彩色多普勒联合超声血管造影及数字减影技术对患者动静脉内瘘通畅/狭窄判定情况无明显差异(P>0.05)。结论:彩色多普勒联合超声血管造影对各型自体动静脉内瘘狭窄血管都能进行精确的评估,可以用于持续监测,对患者身体损伤较小,术后并发症发生率也很低,有重要的临床意义。

Objective:To discuss the evaluation of color Doppler combined with ultrasound angiography before and after PTA treatment of autologous arteriovenous stenosis and the clinical effect comparison.Methods:From September 2019 to September 2020,86 patients with long-term hemodialysis in our hospital who underwent PTA surgery due to autologous arteriovenous fistula stenosis were selected as the study subjects.Color Doppler ultrasound combined with angiography were used to conduct long-term evaluation of the hemodialysis vessels of the patients,and corresponding diagnosis and treatment were carried out.The results were compared with that of digital subtraction angiography to evaluate the hemodialysis vessels,and relevant data were statistically analyzed.Results:After surgery,the vascular diameter value of all types of patients increased to a certain extent,and the blood flow was also significantly improved,with statistically significant differences before and after surgery(P<0.05).Color Doppler ultrasound combined with angiography and digital subtraction angiography showed no significant difference in the determination of arteriovenous fistula patency/stenosis in patients(P>0.05).Conclusion:Color Doppler combined with ultrasound angiography can be used for accurate evaluation of all types of autologous arteriovenous fistula stenosis,which can be used for continuous monitoring,with less physical injury to patients and a low incidence of postoperative complications.Therefore,it is of important clinical significance.
