经阴道彩色多普勒超声在异位妊娠诊断中的临床应用价值

The Clinical Value of Transvaginal Color Doppler Sonography in the Diagnosis of Ectopic Pregnancy

目的:评价经阴道彩色多普勒超声在异位妊娠诊断中的临床应用价值。方法:选取2018年2月~2019年6月沈阳市第九人民医院收治的80例疑似异位妊娠患者为研究对象,所有研究对象均于术前开展经腹部、经阴道彩色多普勒超声检查,并以病理诊断结果作为金标准,对比经腹部、经阴道超声诊断的符合率、灵敏度、特异性。结果:病变检出率相比,经腹部超声低于经阴道超声,差异有统计学意义(P<0.05)。手术病理确诊为异位妊娠者70例,经腹部超声确诊异位妊娠者55例、经阴道超声确诊异位妊娠者67例。诊断符合率相比,经腹部超声的低于经阴道超声,差异有统计学意义(P<0.05);经腹部彩色多普勒超声诊断的灵敏度、特异性分别为87.3%、58.8%,低于经阴道彩色多普勒超声的98.5%、83.3%,差异有统计学意义(P<0.05)。结论:在异位妊娠诊断中,经阴道彩色多普勒超声的应用,不仅可以提高检出率,还可提高诊断符合度,且灵敏度较高,可为临床诊断提供有效的参考依据。

Objective:To evaluate the clinical value of transvaginal color Doppler sonography(TVCDS)in the diagnosis of ectopic pregnancy(EP).Methods:80 patients with suspected EP admitted to Shenyang Ninth People’s Hospital from February 2018 to June 2019 were selected as the study objects.All the subjects were examined by transabdominal(TACDS)and TVCDS before operation,and the pathological diagnosis results were taken as the gold standard to compare the coincidence rate,sensitivity and specificity of TACDS and TVCDS diagnosis.Results:Compared with the detection rate of lesions,TACDS was lower than TVCDS,the difference was statistically significant(P<0.05).There were 70 cases of EP confirmed by operation and pathology,55 cases of EP confirmed by TACDS and 67 cases of EP confirmed by TVCDS.The diagnostic accuracy of TACDS was significantly lower than that of TVCDS(P<0.05);The sensitivity and specificity of TACDS were 87.3%and 58.8%respectively,which were lower than that of TVCDS(98.5%and 83.3%,P<0.05).Conclusion:In the diagnosis of EP,the application of TVCDS can not only improve the detection rate,but also improve the diagnostic accuracy,and the sensitivity is high,which can provide an effective reference for clinical diagnosis.
