经阴道超声对剖宫产瘢痕处妊娠的诊断价值

The Value of Transvaginal Ultrasound in the Diagnosis of Cesarean Scar Pregnancy

目的:探索经阴道超声对剖宫产瘢痕处妊娠的诊断价值。方法:在2018年6月~2019年6月选取96例疑似剖宫产瘢痕妊娠者为此次试验对象,均采用阴道超声诊断,随后分析超声在妊娠类型(孕囊型、宫腔型、包块型)中诊断正确率以及诊断价值。结果:阴道超声对孕囊型诊断正确率为96.00%,对宫腔型诊断正确率为97.56%,对包块型诊断正确率为94.12%。同时阴道超声诊断对剖宫产瘢痕妊娠的敏感度为96.39%,特异度为92.31%,漏诊率为3.61%,误诊率为7.69%。超声影像学表现:宫腔内部不存在妊娠囊,宫颈形态正常,子宫切口位置和妊娠囊回声信号由低信号转变为紊乱,妊娠位置血流信号表现为低速低阻型流速曲线。结论:在判断剖宫产瘢痕妊娠时,运用阴道超声检查,诊断正确率较高。

Objective:To explore the diagnostic value of transvaginal ultrasound in cesarean section scar pregnancy.Methods:From June 2018 to June 2019,96 cases of pregnancy with suspected cesarean section scars were selected as the test subjects.All of them were diagnosed by transvaginal ultrasound,and then the diagnostic accuracy and diagnostic value of ultrasound in pregnancy types(gestational sac,intrauterine cavity and mass)were analyzed.Results:The diagnostic accuracy of vaginal ultrasound was 96.00%,97.56%and 94.12%respectively.Meanwhile,the sensitivity,specificity,missed diagnosis rate and misdiagnosis rate of cesarean scar pregnancy were 96.39%,92.31%,3.61%and 7.69%respectively.Ultrasonographic findings:There was no pregnancy capsule in the uterine cavity,the cervical morphology was normal,the location of the uterine incision and the echo signal of the pregnancy capsule changed from low signal to disorder,and the blood flow signal at the pregnancy location showed a low speed and low resistance velocity curve.Conclusion:In the judgment of cesarean scar pregnancy,using vaginal ultrasound,the diagnosis accuracy is higher.
