观察宫腔镜下电切术联合药物控释性节育环治疗黏膜下子宫肌瘤的效果

To Observe the Effect of Hysteroscopic Electrotomy Combined with Drug-Controlled-Release Contraceptive Ring in the Treatment of Submucous Myoma

目的:探讨宫腔镜下电切术联合药物控释性节育环治疗黏膜下子宫肌瘤的效果。方法:选取(2017年1月~2019年1月)本院收治的90例黏膜下子宫肌瘤患者,对照组(n=45)接受宫腔镜下电切术治疗,观察组(n=45)在对照组基础上接受药物控释性节育环治疗,对比两组患者治疗前后Hb水平、子宫体积、月经量以及并发症和复发情况。结果:治疗后3、6个月观察组Hb水平(110.24±10.54)g/L、(118.41±10.24)g/L明显高于对照组(95.24±10.21)g/L、(104.25±10.11)g/L;差异具有统计学意义(t=6.8571、6.6010、3.0742、5.0866、8.0984、7.6005,P<0.05)。两组患者均没有出现明显并发症,随访12个月,两组患者复发率2.22%vs11.11%,对比差异明显(χ^(2)=6.3523,P<0.05)。结论:黏膜下子宫肌瘤在宫腔镜下电切术治疗基础上联合药物控释性节育环治疗可有效缩小子宫体积,纠正贫血,降低月经量和复发率。

Objective:To investigate the effect of hysteroscopic electrotomy combined with drug-controlled-release contraceptive ring in the treatment of submucous myoma.Methods:90 patients with submucous myoma in our hospital from January 2017 to January 2019 were selected.The control group(n=45)received hysteroscopic electrosurgical resection.The observation group(n=45)received drug-controlled-release contraceptive ring treatment on the basis of the control group.The Hb level,uterine volume,menstrual volume,complications and recurrence of the two groups were compared before and after treatment.Results:At 3 and 6 months after treatment,the level of Hb in the observation group(110.24±10.54)g/L,(118.41±10.24)g/L was significantly higher than that in the control group(95.24±10.21)g/L,(104.25±10.11)g/L;The difference was statistically significant(t=6.8571,6.6010,3.0742,5.0866,8.0984,7.6005,P<0.05).There was no significant complication in the two groups.After 12 months follow-up,the recurrence rate of the two groups was 2.22%vs 11.11%(χ^(2)=6.3523,P<0.05).Conclusion:The combined treatment of submucosal uterine fibroids with hysteroscopic resection and controlled-release contraceptive ring can effectively reduce the size of the uterus,correct anemia,and reduce menstrual flow and recurrence rate.
