CDK12蛋白在前列腺癌中的表达及与临床病理特征的关系

Expression of CDK12 protein in prostate cancer cells and its relationship with clinicopathological characteristics

目的探讨细胞周期依赖性蛋白12(CDK12)在前列腺癌(PCa)中的表达,并分析其与患者临床病理特征间的关系。方法选取2015年1月至2016年10月本院行前列腺癌根治术的PCa患者89例(PCa组)和行前列腺电切术的良性前列腺增生(BPH)患者50例(BPH组),收集两组术中病理标本,采用免疫组化(HC)染色检测组织中CDK12表达量,并分析CDK12蛋白与PCa患者临床病理特征间的关系。PCa患者根据生存情况将其分为生存组与死亡组先后行cOX单因素及多因素分析PCa患者预后的危险因素。根据病理检查中CDK12蛋白表达结果。将PCa患者分为阳性组与阴性组,给制生存曲线,比较3年生存状况。结果PGa组的CDK12蛋白阳性表达率显著高于BPH组(P<005);PCa患者中阳性组与阴性组的年龄比较,差异无统计学意义(P>005).而前列腺特异抗原(PSA)水平.Gleaso评分及肿瘤分期与CDK12蛋白表达星正相关(r=0.311.0.457.0.441,P<0.05)。89例PCa患者3年期间死亡33例,存活56例。C0x回归模型单因素分析提示,存活组和死亡组的PSA Cleason评分、肿瘤分期及病理组织的CDKI2蛋白表达情况比较,差异有统计学意文(P<0.05),C0X多因素分析提示,PSA水平>20 pg/L.Cleason评分>7分.肿瘤分期T3期、CDK12蛋白阳性表达均是影响PCa患者现后的独立危险因素。CDK12蛋白阳性表达者的3年累积生存率低于阴性表达者,差异有统计学意义(X^(2)=5.912,P<0.05)。结论CDK12蛋白的过度表达可能在PCa发生发展过程中发挥重要作用,且其阳性表达水平可能与PCa感性程度呈正相关,研究CDKI2阳性表达量在评估PCa进展风险,规划临床治疗方案中具有潜在价值.CDKI2表达情况也是影响PCa患者预后的关键因素。

Objective To investigate the expression of celin-dependent kinase 12(CDK12)protein in prostate cancer(PCa)cells and its relationship with clinicopathological characteristics.Methods From January 2015 to October 2016,89 PCa patients underwent radical resection of pros-tate cancer and 50 benign prostate hyperplasia(BPH)underwent transurethral resection of the prostate in our hospital were enolled.The expression of CDK12 in two pathological specimens was detected by immunohistochemistry(IHC)staining,and the relationship between CDK12 protein and clinicopatho-logical characteristics of PCa patients was analyzed.After the operation,89 PCa patients were followed u for 3 years,and were divided into survival group and death group according to the survival status.C0X single factor and multifactor analysis were performed to summarize the risk factors affecting the prognosis of PCa patients.According to the CDK12 protein expression results,89 PCa patients were.divided into positive and negative groups,and survival curves were drawn to compare the 3-year surviv-al status of two groups.Results The positive expression ralte of CDK12 protein in PCa tssuse was sig-nificantly higher than that in BPH tsse(P<0.05).The positive expression level of CDK12 protein had no significant diference among PCa patients with different ages(P<0.05),while had sigificant difference among PCa patients with different prostate specific antigen(PSA)levels,Gleason scores and tumor stages(P<0.05).Correlation analysis indicated that PSA level,Gleason score and tumor stage of PCa patients were positively correlated with positive CDK12 protein expression(P<0.05).Three year telephone and out-patient fllowoup review showed that 33 PCa patients died and 56 sur-vived.COX univariate analysis showed that there were significant differences between the survival group and the death group in PSA,Gleason score,tumor stage,and CDK12 protein expression status(P<0.05).COX mulivariate analysis indicated that PSA level>20μg/L,Gleason score>7,tunor stage of T3,and positive expression of CDK12 protein were all independent risk factors that afected the prognosis of PCa patients.Survival curve found that 3-year suvival rate of patients with CDK12 positive expression was significantly diferent from those of negative expression(x^(2)=5.912,p<0.05).Conclusions Over-expression of CDK12 prolein may play an important role in the ocurrence and development of PCa,and its positive expression level may be positively correlated with the degree of disease malignancy.Therefore,the study of CDK12 positive expression level has potential value in the asessment of the risk of PCa progression and the formulation of clinical treatment plans.Moreover,CDK12 expression is also a key factor afcting the prognosis of PCa patients.
