浸润性膀胱癌组织中BCL-2的表达及对患者复发的预测价值

目的探究浸润性膀胱癌(IBC)组织中B淋巴细胞瘤/白血病-2(BCL-2)的表达及对患者复发的预测价值。方法选择2013年5月至2014年5月在本院收治的93例IBC患者为研究对象,收集其经尿道膀胱肿瘤电切术(TURBT)中切除的IBC癌组织及癌旁正常组织用于本研究。采用实时荧光定量逆转录聚合酶链反应(RT-qPCR)检测IBC癌组织及癌旁正常组织中BCL-2基因相的对表达量,比较不同临床病理特征IBC患者癌组织中BCL-2基因相对表达量,比较不同复发情况IBC患者癌组织中BCL-2基因相对表达量,采用受试者工作特征曲线(ROC)分析BCL-2对IBC患者复发的预测价值。结果IBC癌组织中BCL-2基因相对表达量显著高于癌旁正常组织(P<0.05);不同性别、年龄、肿瘤直径的IBC患者癌组织中BCL-2基因相对表达量比较,差异无统计学意义(P>0.05);不同肿瘤转移、肿瘤分期、肿瘤分级的IBC患者癌组织中BCL-2基因相对表达量比较,差异有统计学意义(P<0.05);复发IBC患者癌组织.中BCL-2基因相对表达量显著高于未复发IBC患者(P<0.05);BCL-2预测IBC复发的ROC曲.线下面积为0.940(95%CI:0.887~0.994),敏感度为83.3%,特异度为96.5%,准确度为89.9%。结论IBC组织中BCL-2呈高表达状态,BCL2表达与IBC患者肿瘤转移、肿瘤分期、肿瘤分级及预后相关,BCL-2对IBC复发有较高的预测价值。

Objective To investigate the expression of B cell lymphoma/leukemia-2(BCL-2)in invasive bladder cancer(IBC)issues and its predictive value of relapse in patients.Methods From May 2013 to May 2014,93 IBC patients underwent TURBT in our hospital were enrolled.The IBC cancer tssues and adjacent normal tissues were collected inl the operaion.The relative expression of BCL-2 gene in IBC tissues and adjacent normal tssues was detected by real-time quantitative reverse transcription polymerase chain reaction(RT-qPCR).The relative expression of BCL-2 gene in IBC tis-sues was compared among patients with different clinical pathological features and different recurrence rate.The receiver operating characteristic curve(ROC)was used to analyze the predictive value of BCL-2 for IBC recurrence.Results The relative expression of BCL-2 gene in IBC cancer tissues was significantly higher than that in adjacent normal tssues(P<0.05).The relative expression of BCL-2 gene in IBC tissues had no different among patients with different gender,age and tumor diameter(P>0.05),while had significant diference among patients with different tumor metastasis,tumor stage and tumor grade(P<0.05).The relative expression level of BCL-2 gene in cancer tissues of patients with recurrence was significantly higher than that of patients without recurrence(P<0.05).The area under the ROC curve,sensitivity,specifeity and accuracy of BCL-2 for predicting IBC recur-rence were 0.940(95%CI:0.887-<0.994),83.3%,96.5%,and 89.9%,respectively.Conclu-sions BCL-2 is highly expressed in IBC cancer tissues,which is associated with tumor metastasis,tumor stage,tumor grade and prognosis,moreover,the BCL-2 has a high predictive value for the re-currence of IBC.
