摘要
目的探讨尿液中足细胞标记蛋白(PCX)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、血管内皮细胞黏附因子(VCAM-1)联合检测对糖尿病肾病的诊断价值。方法选取2018年8月至2019年8月本院收治的糖尿病肾病患者60例设为糖尿病肾病组,糖尿病非肾病患者60例设为糖尿病非肾病组.肾脏病患者60例设为肾脏病组.同时选取健康体检者60例设为对照组。检测四组受试者尿液中PCX、NCAL、VCAM-1的表达量。用受试者操作特征(ROC)曲线下面积分析尿液中PCX、NCAL、VCAM-1对糖尿病肾病的诊断价值。结果糖尿病肾病组患者尿液中PCX、NCAL、VCAM-1的表达量均高于糖尿病非肾病组和对照组(P<0.05)。糖尿病肾病组患者和糖尿病非肾病组的尿液中PCX、NCAL、VCAM-1单向检验的灵敏度分别为92%.92%.90%;特异度分别为74%.93%.74%。糖尿病肾病组和对照组的尿液中PCX、NGAL、VCAM-1单向检验的灵敏度分别为95%.95%.93%;特异度分别为92%.89%.90%。糖尿病肾病组和肾脏病组的尿液中PCX、NCAL、VCAM-1单向检验的灵敏度分别为98%.93%、88%;特异度分别为88%.87%.83%。尿液中PCX、NCAL、VCAM-1联合对糖尿病肾病的诊断灵敏度为93.33%,特异度为90.83%。结论尿液中PCX、NCAL、VCAM-1的表达量可作为联合诊断糖尿病肾病的潜在标志物。
Objective To explore the diagnostic value of combined detection of PCX,NGAL,and VCAM-1 in urine for diabetic nephropath.Methods From August 2018 to August 2019,60 dia-betic nephropathy patients were selected as diabetic nephropathy group,60 diabetic non-nephropathy patients as diabetic non-nephropathy group,60 renal patients as the kidney group,and 60 healthy physical examination patients as the control group.The expression levels of PCX,NCAL and VCAM-1 in urine of four groups were detected.The area under the ROC curve was used to analyze the diagnostic value of PCX,NGAL and VCAM-1 in urine.Results The expression levels of PCX,NGAL and VCAM-1 in urine of patients with diabetic nephropathy were higher than those of patients with diabetic non-nephropathy and healthy physical examination(P<0.05).The sensitivity of one-way tests of PCX,NGAL and VCAM-1 in urine of patients with diabetic nephropathy and non-diabetic nephropathy were 92%,92%and 90%,respectively;specificity was 74%,93%and 74%,respectively.The one-way tests of PCX,NGAL and VCAM-1 in urine of patients with diabetic nephropathy and control group were 95%,95%and 93%,and the specificities were 92%,89%and 90%,respectively.The sensitivity of one-way tests of PCX,NGAL and VCAM-1 in urine of patients in diabetic nephropathy group and kindnet group were 98%,93%and 88%,and specificity was 88%,87%and 83%,re-spectively.The sensitivity of urine PCX,NGAL,and VCAM-1 in the diagnosis of diabetic nephropathy was 93.33%and the specificity was 90.83%.Conclusions The expressions of PCX,NGAL and VCAM-1 in urine can be used as potential markers for the combined diagnosis of diabetic nephropathy.
