苏州市HIV抗体初筛阳性标本复核及确证试验结果分析

Analysis of recharge and confirmation test results of initial HIV antibody positive samples in Suzhou City

目的 分析苏州市人类免疫缺陷病毒(HIV)抗体初筛阳性标本的复核及确证试验结果,了解确证试验带型分布特征,为艾滋病防治工作提供依据。方法 对苏州市2017年-2019年HIV抗体初筛阳性标本进行复核,复核阳性用WB试验进行确证,整理WB试验结果,用SPSS 20. 0软件进行统计分析,同时对不确定结果者进行随访。结果 在3 231例复核阳性标本中,确证阳性2 963例,不确定结果 193例。确证阳性中,env类带型gp160、gp120阳性率最高,gag类带型p55阳性率最低。WB带型分布中,p31、p55阳性率在不同性别间差异均有统计学意义(P <0. 05);p55、p17、p39阳性率在不同年龄组间、不同标本来源间差异均有统计学意义(P <0. 05)。随访结果中阳转率为87. 6%,单一p24阳转率最低。结论 苏州市HIV初筛阳性率较低,应加强对筛查实验室的质量控制;某些WB带型在不同性别、年龄和标本来源中的差异反映了HIV感染者的人群特征,有助于HIV疫情防控;WB试验不确定结果者阳转率较高,应加强随访。

Objective To analyzxe the results of recharge and confirmatory test of positive samples of human immunodeficiency virus antibody primary screening in Suzhou City,to understand the distrbulion characteristics of confirmatory tests band patterns,and to provide basis for AIDS prevention and control.Methods Recheck the HIV antibody positive samples from 2017 to 2019 in the City of Suzhou.The positive results were confirmed by WB test.The positive and uncertain results of WB tests were sorted out and analyzed by SPSS 20.0.Al the same time,the uncertain resuls were followed up.Results Among 3231 recheck positive specimens,2963 were confrmed positive,and 193 were uncertain.Among the confirmed positive sam-ples,enw band patterns gp160 and gpl20 had the highest positive rate,while gag band patterns p55 had the lowest positive rate.In the distribution of W B band patterns of positive samples,there was statistical significance on the difference of positive rates of p31 and p55 in genders(P<0.05);there was statistieal significance on the diference of positive rales of p55,pl7 and p39 in diferent age groups and diferent sample sources(P<0.05).The rate of positive conversion was 87.6%in the fllow-up results,and the rate of single p24 positive conversion was the lowest.Conclusion The positive rate of HIV primary screen-ing in Suzhou is low,so we should strengthen the quality control of primary screening laboratory;the differences of some WB band pattems in diferent gender,ages and specimen sources releet the population characteristics of HIV infeeted people,which is helpful for HIV epidemic prevention and control;the positive conversion rate of uncertain WB lest results is high,so we should strengthen the follow-up.
