新冠疫情对北京市常见慢病患者药物获得模式的影响分析 被引量:3

Analysis of the Changes of Patterns of Access to Medicines of the Patients with Common Chronic Diseases after Experiencing the COVID-19 Epidemic in Beijing

摘要 目的:探究新型冠状病毒肺炎疫情(以下简称"新冠疫情")对北京市常见慢病患者药物获得模式的影响,并对疫情期间以及常态化防控期间,北京市相关管理部门、各医疗机构和医药相关行业所采取的确保慢病患者药物可及的积极措施进行总结。方法:采用现场患者调查方法,了解经历新冠疫情的北京市常见慢病患者疫情防控常态化后药物获得模式的变化,采用多因素分析方法研究药物获得模式因疫情改变的影响因素。结果:新冠疫情期间,93.5%应答慢病患者认为个人用药需求可以得到满足;92.3%接受隔离或封闭管理的应答慢病患者,其用药需求可以得到满足。疫情常态化期间,接受过隔离或封闭管理的应答慢病患者更可能改变药物获得模式[OR=6.671,95%CI(2.314,19.235),P<0.05]。51~70岁年龄段的应答慢病患者相对于18~50岁年龄段患者,更倾向于保持原有的药物获得模式[OR=0.231,95%CI(0.282,0.646),P=0.005]。结论:新冠疫情期间的防疫措施给慢病患者的药物获得带来了挑战,但在相关管理部门和各医疗机构及医药相关行业的共同努力下,大多数患者的用药需求基本得到保障。疫情防控常态化后,部分慢病患者的药物获得方式发生了变化。应继续推行并完善疫情期间的各项惠民措施,满足慢病患者安全、方便获得药物的需求。 Objective:To explore the impact of the COVID-19 epidemic on changing the patterns of access to medicines of patients with common chronic diseases in Beijing, and to make a summary of the proactive efforts of relevant government agencies, and all levels of healthcare providers, as well as the pharmaceutical sector to secure the access of medicines for chronic diseases in Beijing.Methods:We conducted an on-site patient survey to learn the change of access to medicines of the patients with common chronic diseases in Beijing in the regular response period compared with that before the COVID-19 epidemic period. We also adopted multivariate logistic regression model to analyze the associated factors with the above changes. Results:During the epidemic, access to medicines of 93.5% of the responded outpatients was satisfied;access to medicines of 92.3% of the responded outpatients who had ever experienced quarantine was satisfied. During the regular response period, the experience of quarantine had a positive impact on changing the patterns of access to medicines of the responded outpatients(OR=6.671, 95%CI 2.314 to 19.235, P<0.05). Compared with the responded outpatients with common chronic diseases in 18 to 50 years old, those in 51 to 70 years old were more prone to maintain the previous pattern of access to medicines(OR=0.231, 95%CI 0.082 to 0.646, P=0.005). Conclusion:During the epidemic of COVID-19, various prevention and control measures in Beijing brought challenges of access to medicines to the patients with common chronic diseases. However, the demands of access to medicines of the responded outpatients were generally secured through proactive efforts of all relevant parties. When shifted to the regular response period, the pattern of access to medicines of some of the responded patients changed compared with that before the epidemic. It is necessary to continue the promotion and improvement of the "long-term prescription for patient with chronic diseases", the "home-delivery service of medicines for patient with reduced mobility", and the "online medical consultation service" etc, which were implemented and contributed to secure the access to medicines for patient with chronic diseases during the epidemic.
