一种改进的微量凝集试验检测布鲁氏菌病抗体 被引量:1

Detection of antibody by improved tube agglutination assay for brucellosis

摘要 目的通过试管凝集试验(tube agglutination test,SAT)和微量凝集试验(micro tube agglutination test)两种方法的对比,探求一种简便而实用的布鲁菌病检测方法。方法在实验条件完全相同的情况下,以阳性、阴性标准血清为标准,优化微量凝集试验的反应时间、反应温度。选择2016-2018年确诊的布病患者62例作为阳性组,同期健康人群54例作为阴性组,同时进行SAT和微量凝集试验,以SAT结果作为诊断标准,分析微量凝集试验的灵敏度、特异度等指标,以评价结果的准确性。结果确定21个小时反应最优时间,确定反应最佳温度为37℃。两种方法结果差异无统计学意义(t=0.405,P>0.05)。微量凝集试验的灵敏度为96.77%、特异度为100.00%,约登指数为0.9677、Kappa值为0.84。结论SAT和微量凝集试验结果一致,而且微量凝集试验更简便、节约试验原材料。因此,微量凝集试验更适合在基层布鲁菌病大量样本检测、诊断中推广应用。 Objective Through the comparison of the tube agglutination test(SAT)and the micro-tube agglutination test,a simple and practical method for the detection of brucellosis is explored.Methods Under the same experimental conditions,the reaction time and reaction temperature of the micro tube agglutination test were optimized using a positive and negative standard serum as standards.From 2016 to 2018,62 patients with confirmed brucellosis were selected as the positive group,and 54 health outpatients in the same period were selected as the negative group.SAT and micro tube agglutination test were conducted simultaneously.The SAT results were used as the diagnostic criteria to analyse the sensitivity and specificity of the modified SAT and evaluate the accuracy of the micro tube agglutination test results.Results The optimal reaction time and temperature were 21 hours and 37℃.The result was no statistically significant differences(t=0.405,P>0.05).The sensitivity of the micro-tube agglutination test was 96.77%,the specificity was 100.00%,the Jorden index was 0.9677 and the Kappa value was 0.84.Conclusion The results of SAT and the modified test tube agglutination test are consistent,and the micro tube agglutination test is more convenient and saves serum.Therefore,the micro tube agglutination test is more suitable for the promotion and application in the detection and diagnosis of a large number of samples of brucellosis.
