广州市中老年人高敏C反应蛋白和白细胞与代谢综合征及其组分的相关性研究 被引量:9

Correlation of high sensitivity C-reactive protein and white blood cell count with metabolic syndrome and its components in middle-aged and older people of Guangzhou

摘要 目的了解广州市社区中老年人高敏C反应蛋白(hs-CRP)、白细胞(WBC)与代谢综合征(MS)及其组分的相关性,为防控与炎症相关的高MS风险和疾病负担提供科学参考。方法选取广州中老年人慢性病前瞻性队列研究基线调查2017年11月至2019年12月招募的9 689名50岁或以上广州市中老年人为研究对象,调查并收集一般人口特征指标、腰围、血压、空腹血糖、血脂、WBC和hs-CRP等数据。采用SPSS 22.0软件进行独立样本t检验、χ^(2)检验、协方差分析和多因素非条件logistic回归分析。结果研究对象MS患病率为15.2%,男性(18.4%)高于女性(14.0%),差异有统计学意义(P<0.01)。调整年龄、性别后,MS组hs-CRP浓度和WBC计数均高于无MS组,MS各组分异常组均高于正常组,差异均有统计学意义(P<0.01);且hs-CRP和WBC均随MS异常组分个数的增加而增加,线性趋势检验均有统计学意义(P<0.01)。多因素logistic回归分析结果显示,不管男性或女性,hs-CRP和WBC均与MS的风险存在关联,hs-CRP和WBC Q4组患MS风险均高于Q1组,男性OR值分别为2.63(95%CI:1.92~3.62)和2.20(95%CI:1.62~2.98),女性OR值分别为4.27(95%CI:3.25~5.60)和3.61(95%CI:2.82~4.62);全人群、男性和女性MS患病风险随hs-CRP和WBC的升高而增加(P趋势<0.01),女性比男性明显。结论 hs-CRP和WBC均与MS及其组分存在关联,随着hs-CRP和WBC的增加,患MS的风险也增加。 Objective To understand the correlation of high sensitivity C-reactive protein(hs-CRP) and white blood cell count(WBC) with metabolic syndrome(MS) and its components in middle-aged and older people of Guangzhou,and to provide the scientific reference for prevention and control of higher MS risk and disease burden related to inflammation. Methods From November 2017 to December 2019,a total of 9 689 middle-aged and elderly people (≥50 years old)in the prospective Cohort study were selected as the subjects. Data on demographic characteristics,waist circumference,blood pressure,fasting plasma glucose,blood lipid,WBC and hs-CRP were investigated and collected. The t test,χ^(2) test,covariance analysis and multivariable logistic regression were used to analyze the data,the used software was SPSS 22.0. Results MS morbidity of subjects was 15.2%,MS morbidity(18.4%) of males was higher than that(14.0%) of females(P<0.01). After adjusting age and gender,hs-CRP concentration and WBC count in MS group were significantly higher than those in non-MS group,the subgroups with anomalous components in MS group were significantly higher those in normal group(P<0.01). The hs-CRP concentration and WBC count increased with the number of the subgroups with anomalous components(P<0.01). The multivariable logistic regression showed that hs-CRP and WBC were associated with MS risk in male or female,MS risk in hs-CRP and WBC Q4 group was higher than that in Q1 group,OR volumes of males were 2.63(95%CI:1.92~3.62) and 2.20(95%CI:1.62~2.98),OR volumes of females were 4.27(95%CI:3.25~5.60) and 3.61(95%CI:2.82~4.62);MS risk in all subjects,males and females increased with hs-CRP and WBC(P<0.01),as compared with males,female was more obvious. Conclusion The hs-CRP and WBC are positively associated with the MS and its components,the MS risk increases with hs-CRP and WBC.
