血府逐瘀汤口服联合苦参汤熏洗对肛瘘患者术后疼痛及创面愈合的影响

Effect of Xuefu Zhuyu decoction combined with Kushen decoction fumigation on postoperative pain and wound healing in patients with anal fistula

目的探讨血府逐瘀汤口服联合苦参汤熏洗对肛瘘患者术后疼痛及创面愈合的影响。方法抽取2017年2月至2020年8月新乡市第二人民医院收治的行低位单纯性肛瘘切除术的肛瘘患者80例,根据随机数字表法将患者分为观察组和对照组,每组40例。对照组术后口服血府逐瘀汤,观察组在对照组治疗基础上给予苦参汤熏洗。比较两组临床疗效、疼痛程度、创面愈合情况以及不良反应发生情况。结果观察组治疗总有效率为95.00%(38/40),高于对照组的85.00%(34/40),差异未见统计学意义(P>0.05);术后第14天,观察组视觉模拟评分法(VAS)评分为(1.06±0.62)分,低于对照组的(1.50±0.68)分,P<0.05;观察组创面肉芽、创面渗液评分低于对照组(P<0.05);观察组创面愈合时间为(17.62±0.60)d,短于对照组的(20.62±3.88)d,P<0.05。治疗期间两组均未出现明显的不良反应。结论对低位单纯性肛瘘切除术的肛瘘患者予以血府逐瘀汤口服联合苦参汤熏洗,能减轻患者术后疼痛程度,缩短创面愈合时间,提高创面愈合率,且不会增加用药不良反应,安全性较高。

Objective To investigate the effects of Xuefu Zhuyu decoction combined with Kushen decoction fumigation on postoperative pain and wound healing in patients with anal fistula.Methods Eighty patients with anal fistula treated in the Second People’s Hospital of Xinxiang from February 2017 to August 2020 were selected.All patients underwent low simple anal fistulectomy.Eighty patients were divided into observation group and control group according to random number table method,with 40 cases in each group.The control group was given Xuefu Zhuyu decoction orally after surgery;the observation group was fumigated with Kushen decoction based on the treatment of the control group.The clinical efficacy,pain degree,wound healing and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 95.00%(38/40),which was higher than 85.00%(34/40)of the control group,but the difference between the two groups was not statistically significant(P>0.05).On the 14th day after operation,the visual analogue scale(VAS)score of the observation group was 1.06±0.62,lower than the 1.50±0.68 of the control group(P<0.05);the scores of granulation and wound exudate in the observation group were lower than those in the control group(P<0.05).The wound healing time of the observation group was(17.62±0.60)d,shorter than the(20.62±3.88)d of the control group(P<0.05).There was no obvious adverse reaction in neither group during the treatment.Conclusions Xuefu Zhuyu decoction combined with Kushen Decoction fumigation can reduce postoperative pain,shorten wound healing time,improve wound healing rate in patients with anal fistula after simple anal fistula resection,without increasing adverse effects.
