sdLDL与apoA1、apoB、LPa水平在冠心病临床分型严重程度评估中的应用价值

摘要 目的探讨冠心病患者小而密低密度脂蛋白(small dense low-density lipoprotein,sdLDL)与载脂蛋白A1(apolipoprotein A1,apoA1)、载脂蛋白B(apolipoprotein B,apoB)、脂蛋白a(lipoprotein a,LPa)水平对临床分型严重程度的评估价值。方法选取2020年1月至6月本院收治的72例冠心病患者为冠心病组,同期健康体检者35例为健康体检组。冠心病组男44例,女28例,年龄(67.01±6.42)岁;健康体检组男23例,女12例,年龄(65.43±5.27)岁。将72例冠心病患者按照临床分型分为稳定型心绞痛26例,不稳定型心绞痛25例,急性心肌梗死21例。检测研究对象sdLDL与apoA1、apoB、LPa水平,分析并讨论各组差异。结果冠心病组sdLDL、apoB、LPa水平分别为(1.53±0.32)mmol/L、(1.17±0.27)g/L、(237.16±40.29)g/L,均高于健康体检组的(0.92±0.19)mmol/L、(0.78±0.22)g/L、(117.06±28.20)g/L,apoA1为(0.99±0.23)g/L,低于健康体检组的(1.24±0.35)g/L,差异均有统计学意义(均P<0.05)。sdLDL、apoB、LPa随着临床分型严重程度加重而升高,apoA1随着临床分型严重程度上升而下降,差异均有统计学意义(均P<0.05)。结论冠心病患者sdLDL、apoA1、apoB、LPa水平与临床分型严重程度相关,具有较高评估价值。 Objective To explore the values of small dense low-density lipoprotein(sdLDL),apolipoprotein A1(apoA1),apolipoprotein B(apoB),and lipoprotein a(LPa)levels in evaluating the severity of clinical classification in patients with coronary heart diseases(CHD).Methods A total of 72 patients with CHD(CHD group)and 35 healthy people(healthy group)in our hospital from January to June 2020 were selected.There were 44 males and 28 females in the CHD group,with the age of(67.01±6.42)years old;there were 23 males and 12 females in the healthy group,with the age of(65.43±5.27)years old.Seventy-two patients with coronary heart diseases were divided into 3 groups according to the clinical classification,including 26 patients with stable angina pectoris,25 patients with unstable angina pectoris,and 21 patients with acute myocardial infarction.The levels of sdLDL,apoA1,apoB,and LPa were detected,and the data of all groups were analyzed and discussed.Results The levels of sdLDL,apoB,and LPa in the CHD group were(1.53±0.32)mmol/L,(1.17±0.27)g/L,(237.16±40.29)g/L,which were higher than those in the healthy group[(0.92±0.19)mmol/L,(0.78±0.22)g/L,(117.06±28.20)g/L],the level of apoA1 in the CHD group was lower than that in the healthy group[(0.99±0.23)g/L vs.(1.24±0.35)g/L],with statistically significant differences(all P<0.05).In the CHD group,sdLDL,apoB,and LPa increased with the severity of clinical classification,while apoA1 decreased with the severity of clinical classification(all P<0.05).Conclusion The levels of sdLDL,apoA1,apoB,and LPa in patients with CHD are correlated with the severity of clinical classification,which have high evaluation values.
