联合临床触诊加摄局部点压摄影在乳腺疾病诊断中的价值

Value of Clinical Palpation and Local Spot Compression Mammography in Diagnosing of Breast Diseases

目的:探讨常规乳腺摄影后联合触诊加摄局部点压摄影对乳腺疾病的诊断价值。方法:常规乳腺摄影后,对40例触诊异常或不能鉴别良恶性的病例加摄局部点压摄影,获得的图像结合触诊特点进行分析诊断,并对比评价常规摄影与局部点压摄影对疾病的诊断。结果:联合触诊加摄的局部点压摄影病变全部检出,远高于常规摄影的病变检出率(80%),且正确诊断率(87.5%)也明显高于常规摄影(60.9%)。40例病例中,其中4例因病灶位置较高较深、紧贴胸壁未能显示完全,局部点压后显示基本满意,3例局灶性非对称致密影经局部点压后显示小肿块伴边缘短小毛刺,结合触诊特点诊断更加明确。结论:在常规乳腺摄影的基础上联合临床触诊加摄局部点压摄影可以显著提高乳腺病变的检出率和正确诊断率。

Objective:To explore the diagnostic value of clinical palpation and local spot Compression mammography after routine mammography for breast diseases.Methods:After routine mammography,40 cases with abnormal palpation or unable to identify benign and malignant cases were added with local spot compression mammography.The images obtained were analyzed and diagnosed in combination with palpation characteristics,and the diagnosis of the disease by routine mammography and local spot compression mammography was compared and evaluated.Results:The detection rate of clinical palpation and local spot compression mammography(100%)is much higher than that of routine mammography(80%),and the correct of diagnosis rate(87.5%)was also significantly higher than that of routine mammography(60.9%).In 40 cases,4 of them failed to show the focus completely because of its high or deep position or close to the chest wall,which was basically satisfactory after the local spot compression mammography.3 cases of focal asymmetric dense shadow showed small mass with short burr on the edge after local spot compression mammography.The diagnosis was more clear combined with palpation characteristics.Conclusion:On the basis of routine mammography combined with clinical palpation and local spot compression mammography can significantly improve the detection rate and correct diagnosis rate of breast lesions.
