支气管哮喘急性发作期患儿治疗后IFN-γ、IL-1β及IgE变化研究 被引量:8

摘要 目的分析特布他林联合布地奈德雾化吸入治疗对支气管哮喘急性发作期患儿伽马干扰素(IFN-γ)、白细胞介素-1β(IL-1β)及免疫球蛋白E(IgE)的影响。方法收集本院2018年6月至2020年6月收治的300例支气管哮喘急性发作患儿的临床资料。根据不同治疗方法分为对照组(n=148例,常规综合治疗)与研究组(n=152例,特布他林联合布地奈德雾化吸入治疗),比较两组临床疗效、临床症状与体征消失时间、肺功能、IFN-γ、IL-1β、IgE及不良反应发生情况。结果对照组治疗后总有效率为80.41%,显著低于研究组的92.11%,差异有统计学意义(P<0.05)。研究组咳嗽、喘憋、哮鸣音及湿罗音消失时间明显短于对照组,差异有统计学意义(P<0.05)。治疗后,两组肺功能指标用力吸气肺活量(FVC)、第一秒最大呼气量占预计值的百分比(FEV1%Pred)、呼气流量峰值(Peak expiratory flow,PEF)水平及血清因子IFN-γ均较治疗前显著上升,IL-1β、IgE水平显著下降,且研究组上述因子改善更为明显,差异有统计学意义(P<0.05)。两组均未发生其他严重不良反应。结论特布他林联合布地奈德雾化吸入治疗支气管哮喘急性发作期患儿疗效明确,可减轻机体炎症反应,改善患儿肺功能,且安全性较高。 Objective To analyze the effect of terbutaline combined with budesonide aerosol inhalation therapy on interferon gamma-γ(IFN-γ),interleukin-1β(Interleukin,IL-1β)and immunoglobulin E(Immunoglobulin E,IgE)in children with acute exacerbation of bronchial asthma.Methods The clinical data of 300 children with acute exacerbation of bronchial asthma admitted in this hospital from June 2018 to June 2020 were collected.According to different treatment methods,they were divided into a control group(n=148 cases,conventional comprehensive treatment)and a study group(n=152 cases,terbutaline combined with budesonide aerosol inhalation therapy).The clinical efficacy,disappearance time of clinical symptoms and signs,lung function,IFN-γ,IL-1β,IgE,and adverse reactions were compared between the two groups.Results The total effective rate after treatment in the control group was 80.41%,which was significantly lower than the 92.11%in the study group,and the difference was statistically significant(P<0.05).The disappearance time of cough,dyspnea,wheezing and moist rales in the study group was significantly shorter than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the lung function indexes forced vital capacity(FVC),the percentage of forced expiratory volume in one second to the predicted value(FEV1%Pred),peak expiratory flow(PEF)and serum factor IFN-γincreased in the two groups,and IL-1βand IgE levels decreased significantly,the above factors in the study group improved more significantly,and the difference was statistically significant(P<0.05).No other serious adverse reactions occurred in the two groups.Conclusion Terbutaline combined with budesonide aerosol inhalation therapy has a clear curative effect in the treatment of children with acute bronchial asthma,which can reduce the body’s inflammatory response and improve the lung function of children with high safety.
