摘要
目的探讨应用立体定向微创手术治疗脑胶质瘤患者的临床疗效及对神经功能、日常生能力的影响。方法选取62例脑胶质瘤患者,采用随机数字表法分为两组,每组各31例。对照组和观察组分别实施常规开颅手术和立体定向微创手术。对比两组临床疗效、神经功能、日常生活能力及神经肽水平[β-内啡肽(β-EP)和精氨酸升压素(AVP)水平]。结果观察组临床总有效率(96.77%)高于对照组(74.19%),治疗后日常活动能力量表(ADL)评分为(79.74±3.53)分,高于对照组的(54.15±2.78)分,神经功能缺损程度量表(NIHSS)评分为(12.22±2.57)分,低于对照组的(16.90±2.65)分,β-EP(82.20±4.23)ng/l、AVP(15.32±3.84)ng/l,高于对照组的(63.33±4.37)ng/l、(11.17±8.19)ng/l,复发率(0.00%)低于对照组(22.58%)差异有统计学意义(P<0.05)。结论与常规开颅手术相比,立体定向微创手术应用于脑胶质瘤患者中取得了较佳的效果,神经功能和日常生活得到明显改善,脑脊液神经肽也可有效恢复,且无复发,利于预后。
Objective To investigate the effect of stereotactic minimally invasive surgery on clinical efficacy,nerve function and daily life of patients with glioma.Methods 62 patients with glioma were selected,Using the random number table method,each group had 31 cases.Routine craniotomy and stereotactic minimally invasive surgery were performed in the control group and the observation group respectively.The clinical efficacy,neurological function,daily life ability and neuropeptide level were compared between the 2 groups[β-endorphins(β-EP)and arginine vasopressin(AVP)levels].Results The total effective rate in the observation group(96.77%)was higher than that in the control group(74.19%),Post-treatment daily activity scale(ADL)score(79.74±3.53),higher than(54.15±2.78)points in the control group,Neurological impairment scale(NIHSS)score(12.22±2.57),Below(16.90±2.65)points in the control group,β-EP(82.20±4.23)ng/l、AVP(15.32β-EP(82.20±3.84)ng/l,ng/l,(63.33±4.37)ng/l、(11.17±8.19)higher than controls recurrence rate(0.00%)was lower than that of the control group(22.58%)(P<0.05).Conclusion Compared with conventional craniotomy,stereotactic minimally invasive surgery has achieved better results in glioma patients,nerve function and daily life have been significantly improved,cerebrospinal fluid neuropeptides can also be effectively recovered,and no recurrence.It is beneficial to prognosis.
