展筋活血方穴位贴敷联合针刺治疗膝骨关节炎70例疗效观察

Clinical observation of acupoint application of Zhanjin Huoxue Prescription combined acupuncture on knee osteoarthritis in 70 cases

目的观察展筋活血方穴位贴敷联合针刺治疗膝骨关节炎(KOA)的临床疗效。方法将70例KOA患者按照随机数字表法分为2组。对照组35例予塞来昔布胶囊口服,治疗组35例在对照组治疗基础上予展筋活血方穴位贴敷联合针刺治疗。2组均2周为1个疗程,共治疗2个疗程。2组治疗前后比较疼痛视觉模拟评分(VAS)、西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评分及膝关节运动功能Lysholm评分变化,并统计疗效。结果治疗组总有效率94.29%(33/35),对照组总有效率77.14%(27/35),治疗组疗效优于对照组(P<0.05)。治疗2、4周后2组疼痛VAS均较本组治疗前降低(P<0.05),且治疗2、4周后治疗组均低于对照组同期(P<0.05)。治疗2、4周后2组WOMAC骨关节炎指数各项评分及总分均较本组治疗前降低(P<0.05),且治疗2、4周后治疗组均低于对照组同期(P<0.05)。治疗2、4周后2组Lysholm各项评分均较本组治疗前升高(P<0.05),且治疗2、4周后治疗组均高于对照组同期(P<0.05)。结论展筋活血方穴位贴敷联合针刺治疗KOA疗效确切,能有效降低患者疼痛程度,缓解临床症状,改善患者膝关节运动功能,从而提高生活质量。

Objective To observe the clinical effect of acupoint application of Zhanjin Huoxue Prescription combined with acupuncture on knee osteoarthritis(KOA).Methods Totally 70 patients with KOA were randomly divided into the control and treatment group,with 35 cases in each.The control group was treated with oral celecoxib capsule,the treatment group were treated with acupoint application of Zhanjin Huoxue Prescription combined with acupuncture on the basis of the control group.The two groups had 1 session in 2 weeks,with a total of 2 sessions,aim to observe the changes of the pain visual analogue score(VAS),Western Ontario and McMaster University(WOMAC)osteoarthritis index score,and knee joint motor function score(Lysholm)before and after treatment between the two groups.Results The total effective rate in the treatment group was better than that in the control group[94.29%(33/35)vs 77.14%(27/35),(P<0.05),respectively].After 2 or 4 weeks of treatment,the scores of VAS and WOMAC osteoarthritis index score in the two groups were significantly lower than those before treatment(P<0.05),and which in the treatment group were lower than the control group(P<0.05);the Lysholm scores in the two groups were significantly higher than those before treatment(P<0.05),and which in the treatment group were higher than those in the control group(P<0.05).Conclusion It was significantly effect of acupoint application of Zhanjin Huoxue Prescription combined with acupuncture on KOA,which can effectively reduce the pain degree of patients,relieve the clinical symptoms,improve the joint movement function of patients,and thus improve the quality of life.
