补肾活血方联合碳酸钙D_(3)片、阿仑膦酸钠片治疗老年骨质疏松性髋部骨折全髋关节置换术后临床研究

Effect of Bushen Huoxue Recipe combined with calcium carbonate d3 tablets and alendronate tablets in the treatment of elderly osteoporotic hip fracture after total hip arthroplasty

目的观察补肾活血方联合碳酸钙D_(3)片、阿仑膦酸钠片治疗老年骨质疏松性髋部骨折全髋关节置换术后的临床疗效。方法将100例老年骨质疏松性髋部骨折全髋关节置换术后患者按照随机数字表法分为2组,对照组50例予碳酸钙D 3片和阿仑膦酸钠片治疗,治疗组50例在对照组基础上加用补肾活血方治疗。2组均治疗12周,比较2组治疗前后疼痛视觉模拟评分(VAS)、骨密度及骨代谢指标[包括血清Ⅰ型前胶原氨基端前肽(PINP)和Ⅰ型胶原交联羧基末端肽(CTX-Ⅰ)]水平变化情况。结果2组治疗后疼痛VAS均明显降低(P<0.05),骨密度均明显升高(P<0.05),且治疗组治疗后改善优于对照组(P<0.05)。2组治疗后骨代谢指标PINP和CTX-Ⅰ水平与本组治疗前比较均明显降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。结论补肾活血方联合碳酸钙D 3片、阿仑膦酸钠片治疗骨老年质疏松性髋部骨折全髋关节置换术后临床疗效确切,可明显缓解患者疼痛症状,提高骨密度,改善骨代谢水平。

Objective To observe the effect of Bushen Huoxue Recipe combined with calcium carbonate d3 tablets and alendronate tablets in the treatment of elderly osteoporotic hip fracture after total hip arthroplasty. Methods Totally 100 elderly patients with osteoporotic hip fracture after total hip arthroplasty were randomly divided into treatment group and control group,with 50 cases in each group. The control group was given oral calcium carbonate d3 tablets and alendronate tablets,and the treatment group was additionally treated with Bushen Huoxue Recipe on the basis of the control group. The course of treatment in the both groups was 12 weeks,the score of visual analogue scale(VAS) for pain,the index of femoral bone mineral density and bone metabolism markers [(serum N-terminal propeptide of type Ⅰ procollagen(PINP) and C-telopeptide of type Ⅰ collagen(CTX-Ⅰ) ] were compared between the two groups before and after treatment. Results After treatment,the scores of VAS for painand the index of femoral bone mineral density in the two groups were significantly lower or hihger than that before treatment(P <0. 05),and which in the treatment group was significantly better than the control group(P < 0. 05);the bone metabolism markers(PINP,CTX-Ⅰ) in the two groups was significantly decreased after treatment(P < 0. 05),which in the treatment group was significantly lower than the control group(P < 0. 05). Conclusion The addition of Bushen Huoxue Recipe on the basis of calcium carbonate d3 tablets and alendronate tablets can relieve pain of elderly patients with osteoporotic hip fracture after surgery,increase bone mineral density and improve bone metabolism.
