利多卡因局部麻醉联合右美托咪定镇静在儿童乳磨牙根管治疗中的应用

The application of local anesthesia with lidocaine combined with dexmedetomidine for calm of the children undergoing root canal therapy of deciduous molars

目的:探索利多卡因局部麻醉联合右美托咪定镇静在儿童乳磨牙根管治疗中应用的有效性。方法:用随机数字表法纳入门诊乳磨牙根管治疗的儿童60名,随机分为2组(n=30)。实验组患儿在使用利多卡因局部麻醉后联合静脉泵注右美托咪定(0.75μg/kg);对照组患儿在使用利多卡因局部麻醉后联合静脉泵注等容积乳酸钠林格氏液。采用双盲法,手术期间对患儿进行Ramsay评分,术后进行VAS疼痛评分,比较2组的麻醉有效率和术后并发症发生率。结果:实验组和对照组Ramsay评分分别为4.7±0.7和1.5±0.5(P<0.001);VAS分别为1.5±0.5和3.7±1.3(P<0.001),术后不良反应发生率分别为13.3%和16.7%(P>0.05),麻醉有效率分别为93.3%和67%(P<0.05)。结论:右美托咪定的使用安全稳定、疗效可靠,不会增加术后并发症的发生率。

Objective:To study the effects of local anesthesia with lidocaine combined with dexmedetomidine for calm of the children undergoing root canal therapy of deciduous molars.Methods:60 children underwent root canal therapy of deciduous molars were randomly devidid into 2 groups(n=30)and treated by local anesthesia with lidocaine combined with dexmedetomidine(0.75μg/kg)for calm(test group)and lidocaine combined with same volume of Lactated Ringer's solution(control group)respectively.Double-blind study was used.Ramsay scores were recorded during operations,VAS scores,anesthesia effectiveness and postoperative complications were analyzed after opaertion.Results:The average score of Ramsay in test and control group was 4.7±0.7 and 1.5±0.5(P<0.001),the VAS 1.5±0.5 and 3.7±1.3,respectively(P<0.001);the indidence of postoperative complications was 13.3%and 16.7%(P>0.05),the effectiveness of an esthesia was 93.3%and 67%,respectively.Conclusion:Local anesthesia with lidocaine combined with dexmedetomidine is safe and effective for calm of the children undergoing root canal therapy of deciduous molars.
